摘要
目的:探讨机器人辅助腹腔镜下根治性手术对早期宫颈癌患者术后盆底功能的影响。方法:选取2020年1月—2023年11月武汉市第一医院收治的114例早期宫颈癌患者为研究对象,采用随机数表法将其分为对照组(n=57)和研究组(n=57)。对照组采用传统腹腔镜根治性手术治疗,研究组采用机器人辅助腹腔镜下根治性手术治疗。比较两组患者的手术相关指标、应激指标、疼痛程度、术后盆底功能障碍发生率及生活质量。结果:与对照组相比,研究组的术中出血量更少,手术时间、拔除引流管时间、胃肠功能恢复时间及住院时间更短(P<0.05)。两组患者术后肾上腺素、去甲肾上腺素和皮质醇水平比较,研究组更低(P<0.05)。研究组术后1 d、3 d、5 d及7 d的NRS评分及术后盆底功能障碍发生率较对照组更低(P<0.05),术后世界卫生组织生存质量测定简表量表(WHOQOL-BREF)评分更高(P<0.05)。结论:机器人辅助腹腔镜下根治性手术可降低早期宫颈癌患者的术中出血量、生理应激、疼痛程度及术后盆底功能障碍发生率,缩短手术及恢复时间,提高患者术后生活质量。
Objective:To explore the effect of robot-assisted laparoscopic radical surgery on postoperative pelvic floor function in early cervical cancer patients.Methods:A total of 114 patients with early cervical cancer who were treated in Wuhan NO.1 Hospital from January 2020 to November 2023 were included in this study.The patients were and randomly divided into the control group(n=57)and the study group(n=57)using a random number table.Surgical indicators,stress indicators,pain degree,incidence of postoperative pelvic floor dysfunction,and quality of life of patients in the two groups were compared.Results:Compared with the control group,the study group had shorter operative time,drainage tube removal time,gastrointestinal function recovery time,length of hospital stay and less intraoperative bleeding(P<0.05).Postoperative levels of epinephrine(E),norepinephrine(NE),and cortisol(Cor)in the study group was lower than those in the control group(P<0.05).The NRS scores at 1 day,3 days,5 days,and 7 days after surgery and incidence of postoperative pelvic floor dysfunction in the study group were lower than those in the control group(P<0.05).The postoperative World Health Organization Quality of Life Brief Scale(WHOQOL-BREF)score of the study group was higher than that of the control group(P<0.05).Conclusion:Robotassisted laparoscopic radical surgery for early cervical cancer could reduce intraoperative bleeding,relieve physiological stress,lower pain degree and incidence of postoperative pelvic floor dysfunction,shorten operative time and postoperative recovery time,and improve patients’quality of life.
