基于“新木桶理论”的医学技术培训促进中医住院医师提升临床思维能力

摘要 目的构建基于“新木桶理论”的中医住院医师规范化培训中医学技术培训方法,促进中医住院医师提升临床思维能力。方法选取2021年7月—2023年6月在中医培训基地放射科轮转的住院医师为研究对象,包含2019级至2022级。2020级住院医师为观察组,采取“新木桶理论”培训方法,包括调研后分层教学,以培养临床诊断思维能力为主线的师生互动,分析、复盘病例以及小讲课中引入超声、检验和病理等;2019级住院医师作为对照组,采取传统教学方法。分别在次年完成3次放射模拟考核和结业理论考核。对2020级至2022级住院医师进行问卷调查。结果与2019级相比,2020级第1次模拟考成绩及结业专业理论考核成绩明显提高(P<0.01)。59名参与问卷调查的2020级至2022级住院医师中,88%人员希望纳入超声、检验和病理医技培训。结论基于“新木桶理论”的医学技术培训有助于提升中医住院医师的考核成绩及临床思维能力。 Objective To construct a medical technology teaching method based on"New Cannikin Law",promoting the clinical thinking abilities of the residents of Traditional Chinese Medicine in the standardized training.Methods The standardized training residents(Grade 2019 to 2022)who studied in our Department of Radiology from July 2021 to June 2023 were chosen as the subjects.Grade 2020 residents as the experimental group were trained by"New Cannikin Law"teaching method,including stratified teaching based on investigation,teacher-student interaction,the introduction of ultrasound medicine,laboratory medicine and pathology in the analysis and review of cases and lectures.Grade 2019 residents with the traditional teaching way were regarded as the control group.The two groups completed three radiological simulation examinations and the graduation theoretical assessment of the standardized residency training.The residents of Grade 2020 to 2022 only received stratified questionnaire survey.Results Compared with Grade 2019 residents,the first radiological examination score and theoretical assessment score of Grade 2020 were significantly increased(P<0.01).Among the 59 residents in Grade 2021 to 2022 who participated in the questionnaire survey,88%hoped to be included in medical training of ultrasound,laboratory and pathology.Conclusion The medical technology teaching method based on"New Cannikin Law"of standardized training for Traditional Chinese Medicine residents can help improve their assessment scores and their clinical thinking abilities.
