摘要
目的:基于术后计算机体层成像(CT)评估国产机器人辅助全髋关节置换假体定位的精准性。方法:回顾分析2023年3月~2023年7月行国产机器人辅助全髋关节置换术的38例(38髋)患者的临床资料。手术入路均为后外侧入路,术后骨盆及双侧股骨CT检查。测量髋臼杯外展角、髋臼杯前倾角、联合偏心距差及下肢长度差等参数,并与术前规划比较,进行Pearson相关分析。结果:与术前规划相比,术后髋臼杯外展角、髋臼杯前倾角、联合偏心距差、下肢长度差的差值分别为(3.3±2.5)˚、(2.9±2.2)˚、(2.2±1.9)mm、(2.5±2.1)mm。相关系数r分别为0.600、0.820、0.715、0.671,P<0.001。97.4%(37/38)的外展角位于“Lewinnek安全区”,84.2%(32/38)的前倾角位于“Lewinnek安全区”,78.9%(30/38)的联合偏心距差≤5mm,97.4%(37/38)的下肢长度差≤10mm。结论:国产机器人辅助全髋关节置换可较好地实施手术计划,髋臼假体植入具有良好的精准性。
Objective:To investigate the accuracy and precision of prosthesis positioning in domestic robot-assisted total hip arthroplasty through postoperative CT evaluation.Methods:The clinical data of 38 patients(38 hips)who underwent domestic robot-assisted total hip arthroplasty in our hospital from March 2023 to July 2023 were retrospectively analyzed.Posterolateral approach and postoperative CT scan were performed in all cases.Acetabular cup inclination,cup anteversion,combined offset and leg length difference were measured postoperatively and compared with the planed preoperative data.Pearson correlation analysis was used to calculate the relevance.Results:Compared with the preoperative plan,the deviations of acetabular cup inclination,cup anteversion,combined offset difference and leg length difference were(3.3±2.5)˚,(2.9±2.2)˚,(2.2±1.9)mm and(2.5±2.1)mm,respectively.The Pearson correlation coefficient was 0.600(P<0.001),0.820(P<0.001),0.715(P<0.001)and 0.671(P<0.001)respectively.97.4%(37/38)of the inclination angles and 84.2%(32/38)of the anteversion angles were located in the"Lewinnek safe zone"postoperatively.78.9%(30/38)of the combined offset differences were within 5mm and 97.4%(37/38)of the leg length differences were within 10mm.Conclusion:The domestic robot-assisted total hip arthroplasty can satisfactorily carry out the preoperative plan and obtain accurate acetabular cup placement.
