摘要
目的分析全髋关节置换术后患者下肢功能恢复时间的影响因素。方法选取2019年1月至2022年6月海安市中医院收治的因股骨颈骨折或股骨头坏死行全髋关节置换术的172例患者作为研究对象。采用Kaplan-Meier生存分析法统计全髋关节置换术后下肢功能恢复时间,采用单因素和多因素COX回归分析术后患者下肢功能恢复时间的影响因素。结果Kaplan-Meier生存分析显示,术后患者下肢功能恢复的最短时间为3 d,中位时间为8 d(95%CI:7.51~8.49 d)。单因素及多因素COX回归分析结果均显示,年龄较大、手术时间较长、术中出血量多是患者下肢功能恢复时间延长的独立危险因素(P<0.05);股骨颈骨折是患者术后下肢功能恢复时间短的保护因素(P<0.05)。结论年龄较大、手术时间较长、术中出血量多可延长全髋关节术后患者下肢功能恢复时间。
Objective To analyze the factors affecting the recovery time of lower limb walking function after the total hip replacement.Methods A total of 172 patients who underwent total hip replacement due to femoral neck fracture or femoral head necrosis in Haian Hospital of Traditional Chinese Medicine from January 2019 to June 2022 were selected as the study subjects.Kaplan-Meier survival analysis was used to calculate the recovery time of lower limb walking function after total hip replacement,and COX univariate and multivariate regression analysis were used to investigate the factors affecting the recovery speed of lower limb function.Results Kaplan-Meier survival analysis showed that the shortest recovery time of lower limb function was 3 days,the median time was 8 d(95%CI:7.51-8.49).Univariate and multivariate COX regression analysis all showed that older age,longer operation time and larger intraoperative blood loss were independent risk factors for longer recovery time of lower limb function(P<0.05);femoral head necrosis was the factor of shorter recovery time of lower limb function(P<0.05).Conclusion Older age,longer operation time,larger intraoperative blood loss can prolong the recovery time of lower limb function after total hip replacement.
