摘要
目的:观察加味参附颗粒对阳气亏虚、血瘀水停证心肾综合征病人心肾功能的影响。方法:选取2021年1月—2022年10月在广东省第二中医院重症监护室(ICU)住院的心肾综合征病人72例,按照随机数字表法将入选的病人随机分为治疗组和对照组。对照组采用常规西医治疗,治疗组在对照组的基础上加用中药制剂加味参附颗粒。比较两组治疗前后心功能及肾功能的变化情况。结果:治疗组总有效率为83.33%,高于对照组的58.33%,差异有统计学意义(P<0.05)。治疗组不良反应发生率与对照组比较,差异无统计学意义(25.00%与27.78%,P>0.05)。两组治疗前基线水平、心功能指标、肾功能指标比较,差异无统计学意义(P>0.05)。治疗组经治疗后N末端B型利钠肽原(NT-proBNP)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、血肌酐(Scr)、尿素氮(BUN)、血清胱抑素C(CysC)水平低于对照组,左室射血分数(LVEF)高于对照组,差异均有统计学意义(P<0.05)。结论:加味参附颗粒治疗心肾综合征(阳气亏虚、血瘀水停证)可更好地改善病人心功能和肾功能,减轻临床症状,且不会增加不良反应,安全性较好。
Objective:To observe the effect of flavored ginseng granules on cardiac and renal function in cardiorenal syndrome(CRS)patients with yang deficiency and blood stasis and edema.Methods:A total of 72 patients with CRS were selected,and randomly divided into treatment group and control group according to the random number table method,the patients in control group were treated by conventional Western medicine,and the patients in treatment group were treated with traditional Chinese medicine preparations and flavored ginseng granules on the basis of the control group,and the cardiac function and renal function before and after treatment were compared between the two groups.Results:The total effective rate of treatment group was 83.33%,which was higher than that of control group(58.33%),the difference was statistically significant(P<0.05).There was no significant difference in the rate of adverse reactions between the two groups(25.00%vs 27.78%,P>0.05).There were no significant differences in baseline levels,cardiac function indexes,and renal function indexes between the two groups before treatment(P>0.05).After treatment,the levels of N-terminal B-type natriuretic peptide(NT-proBNP),left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),serum creatinine(Scr),urea nitrogen(BUN),and serum cystatin(CysC)were lower in treatment group than those in control group,and left ventricular ejection fraction(LVEF)in treatment group was higher than that in control group,and the differences were statistically significant(P<0.05).Conclusion:The treatment of cardiorenal syndrome with flavored ginseng granules can effectively improve cardiac and renal function,reduce clinical symptoms,and with high safety.
