摘要
纤维软骨栓塞性脊髓病(FCEM)是由供应相应脊髓区域动脉和(或)静脉发生栓塞所引起的脊髓功能障碍综合征。本病例患犬突发右侧偏瘫,结合其临床表现、体格检查、实验室检查、神经学检查和磁共振扫查结果,高度怀疑为纤维软骨栓塞,西医治疗2周后其自主运动部分恢复,能短暂站立但仍无法行走,转诊至中兽医门诊后辨证为肾气虚伴气滞血瘀,对证进行针刺治疗并配以推拿复健,经6次针刺治疗后,该患犬自主运动能力完全恢复,生活质量明显改善,临床疗效显著。本报告对此中西医结合治疗犬纤维软骨栓塞致偏瘫的病例进行介绍,以期为兽医临床工作者提供参考。
Fibrocartilaginous embolic myelopathy(FCEM)is a syndrome of spinal cord dysfunction caused by embolism of the arteries and/or veins supplying the respective spinal cord regions.In this case,the dog suddenly developed right-sided hemiparesis.Based on its clinical symptoms,physical examination,laboratory tests,neurological examination,and magnetic resonance imaging(MRI)results,fibrocartilaginous embolism was highly suspected.After two weeks of Western medical treatment,the dog showed partial recovery of voluntary movement and could stand briefly but was still unable to walk.It was then referred to a traditional Chinese veterinary clinic,where it was diagnosed with kidney qi deficiency accompanied by qi stagnation and blood stasis.Acupuncture treatment and massage rehabilitation were administered according to the diagnosis.After six sessions of acupuncture treatment,the dog fully regained voluntary movement,significantly improving its quality of life with remarkable clinical efficacy.This report introduces the case of treating FCEM-induced hemiparesis in a dog with integrated Chinese and Western medicine,aiming to provide a reference for veterinary practitioners.
