摘要
目的评估数字减影血管造影(DSA)引导下三叉神经半月节球囊压迫术(PBC)治疗原发性三叉神经痛的疗效及相关并发症。方法选取2019年9月至2022年4月武警重庆总队医院收治的120例原发性三叉神经痛患者作为研究对象,患者均在DSA引导下接受PBC治疗,观察患者术后疼痛缓解情况、复发情况及症状(面部麻木、单纯疱疹、复视、耳深部疼痛)发生情况。结果手术一次性穿刺成功率为100.0%,无患者因穿刺引起的严重并发症,术后即刻疼痛缓解118例,延迟缓解2例;复发1例,无死亡病例;术后患侧面部麻木120例,咀嚼肌无力78例,单纯疱疹15例,复视1例,耳深部疼痛5例。结论DSA引导下行卵圆孔穿刺成功率高,PBC疗效显著,无严重并发症,值得临床推广应用。
Objective To evaluate the efficacy and related complications of digital subtraction angiography(DSA)-guided percutaneous balloon compression(PBC)of trigeminal semilunar ganglion in the treatment of primary trigeminal neuralgia.Methods 120 patients with primary trigemi-nal neuralgia admitted to Armed Police Hospital of Chongqing from September 2019 to April 2022 were enrolled as the study subjects,and all the pa-tients were treated with PBC guided by DSA.Postoperative pain relief,recurrence and symptoms(facial numbness,herpes simplex,diplopia,deep ear pain)were observed.Results Among these 120 patients,the success rate of one-time puncture in patients enrolled in this study was 100.0%,and there were no serious complications caused by puncture;there were 118 casesshowed immediate postoperative pain relief,2 cases had delayed relief;1 case showed recurrence of trigeminal neuralgia,there were no patients died in this study.In addition,there were 120 cases of lateral numbness,78 cases of masticatory muscle weakness,15 cases of herpes simplex,1 case of diplopia and 5 cases of earache.Conclusion DSA-guided PBC has high success rate,and PBC has significant effect without serious complications,which is worthy of clinical promotion and application.
