摘要
目的探讨锁骨下动脉狭窄患者椎动脉频谱形态变化及临床意义。方法回顾性分析2019年3月至2022年1月于萍乡市人民医院通过数字剪影血管造影(DSA)检查确诊的82例锁骨下动脉狭窄患者的临床资料,根据DSA评估的患者锁骨下动脉狭窄程度分为A组、B组、C组。根据彩色多普勒超声检查结果,分析3组颅内段、颅外段不同血流频谱特征的相关性。结果Spearman秩相关分析结果显示,A组主要对应颅内段及颅外段血流频谱Ⅰ期形态,B组主要对应颅内段及颅外段血流频谱Ⅱ期形态,C组主要对应颅内段及颅外段血流频谱Ⅲ期形态,患者锁骨下动脉狭窄程度与椎动脉血流频谱形态特征呈正相关(颅内段:r=0.486,P=0.031;颅外段:r=0.491,P=0.030)。结论椎动脉频谱形态变化对判断患者锁骨下动脉狭窄程度具有临床应用价值。
Objective To investigate the morphological changes and clinical significance of vertebral artery spectrum in patients with an artery stenosis.Methods The clinical data of 82 patients with subclavian artery stenosis diagnosed by digital subtraction angiography(DSA)Pingxiang People's Hospital from March 2019 to January 2022 were retrospectively analyzed,the patients were divided into the group A,the group B,and the group C according to the degree of subclavian artery stenosis evaluated by DSA.Based oncolor Doppler ultrasound examination re-sults,the correlation between different blood flow spectrum characteristics of intracranial and extracranial segments in the three groups was analyzed.Results Spearman rank correlation analysis results showed that the group A mainly corresponded to phaseⅠmorphology of intracranial and extracranial blood flow spectra,the group B mainly corresponded to phaseⅡmorphology of intracranial and extracranial blood flow spectra,and the group C mainly corresponded to phaseⅢmorphology of intracranial and extracranial blood flow spectra,the degree of subclavian artery stenosis was positively correlated with morphological characteristics of vertebral artery blood flow spectrum(intracranial segment:r=0.486,P=0.031;extracranial segment:r=0.491,P=0.030).Conclusion Morphological changes of vertebral artery spectrum can help to determine the degree of subclavian artery stenosis.
