摘要
目的探究急性胰腺炎(AP)患者凝血功能相关指标与疾病严重程度的相关性。方法收集2022年1月至2023年8月湖南中医药大学附属长沙市中医医院收治的60例AP患者作为研究对象,根据病情严重程度分为轻症急性胰腺炎(MAP)组和重症急性胰腺炎(SAP)组。另选取同期该院20例健康体检者作为对照组。结果SAP组的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D)水平高于MAP组,凝血酶时间(TT)低于MAP组,差异有统计学意义(P<0.05)。经相关分析及证实,PT、APTT、FIB、D-D与AP病情严重程度呈正相关,TT与病情严重程度呈负相关,差异有统计学意义(P<0.05);经ROC分析,PT、FIB和D-D对SAP具有较优的预测价值。结论监测AP患者凝血功能可在一定程度上反映AP病情严重程度。同时,PT、FIB、D-D对SAP的发生具有较优的预测价值。
Objective To explore the correlation between coagulation function and disease severity in patients with acute pancreatitis(AP).Methods A total of 60 AP patients admitted to Changsha Hospital of Traditional Chinese Medicine to Hunan University of Chinese Medicine from January 2022 to August 2023 were collected as the research objects and divided into the mild acute pancreatitis(MAP)group and the severe acute pancreatitis(SAP)group according to the disease severity.In addition,20 healthy people undergoing physical examination in this hospital during the same period were selected as the control group.Results The prothrombin time(P T),activated partial thromboplas tin time(AP TT),fibrinogen(F IB)level and D-dimer(D-D)level in the SAP group were higher than those in the MAP group,and the thrombin time(TT)in the SAP group was lower than that in MAP group,with statistically significant differences(P<0.05).The correlation analysis confirmed that PT,APTT,FIB and D-D were positively correlated with the severity of AP,while TT was negatively correlated with the severity of AP,with statistically significant differences(P<0.05).ROC analysis showed that PT,FIB and D-D had better predictive value for SAP.Conclusion The coagulation function of AP patients can reflect the severity of AP to some extent.At the same time,PT,FIB and D-D have excellent predictive value for the occurrence of SAP.
