摘要
目的探讨肠内免疫营养治疗在食管癌放疗患者中的应用效果。方法根据营养治疗方法的不同将106例食管癌放疗患者分为常规组(n=53,常规营养治疗)和免疫组(n=53,肠内免疫营养治疗)。比较两组患者的临床疗效、炎性反应指标[降钙素原(PCT)、C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]、营养指标[总蛋白(TP)、转铁蛋白(TRF)、前白蛋白(PAB)、白蛋白(ALB)]、免疫功能指标[免疫球蛋白(Ig)G、IgM、IgA、CD3^(+)及CD4^(+)/CD8^(+)]、不良反应发生情况及预后指标[体重指数、患者主观整体营养状况评估量表(PGSGA)评分及欧洲癌症研究与治疗组织生命质量测定量表(EORTC QLQ-C30)评分]。结果免疫组患者的临床疗效优于常规组,总有效率高于常规组,差异均有统计学意义(P﹤0.05)。治疗后,两组患者CRP、PCT、IL-6、TNF-α、TRF、ALB、PAB、TP水平均高于本组治疗前,免疫组患者CRP、PCT、IL-6、TNF-α水平均低于常规组,TRF、ALB、PAB、TP水平均高于常规组,差异均有统计学意义(P﹤0.05)。治疗后,免疫组患者IgG、IgM、IgA、CD3^(+)水平及CD4^(+)/CD8^(+)均高于常规组,差异均有统计学意义(P﹤0.05)。免疫组患者不良反应总发生率低于常规组(P﹤0.05)。治疗后,两组患者体重指数和EORTC QLQ-C30评分均高于本组治疗前,PG-SGA评分均低于本组治疗前,免疫组患者体重指数和EORTC QLQ-C30评分均高于常规组,PG-SGA评分低于常规组,差异均有统计学意义(P﹤0.05)。结论肠内免疫营养治疗可提高食管癌放疗患者的临床疗效,减轻炎性反应程度,改善营养状态和免疫功能,降低不良反应发生率,进而改善患者预后。
Objective To explore the application effect of enteral immunonutrition therapy in patients with esophageal cancer undergoing radiotherapy.Method A total of 106 patients with esophageal cancer who received radiotherapy were divided into conventional group(n=53,conventional nutritional therapy)and immune group(n=53,enteral immunonutrition therapy)according to different nutritional treatment methods.The clinical efficacy,inflammatory response indexes[procalcitonin(PCT),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)],nutritional indexes[total protein(TP),transferrin(TRF),prealbumin(PAB),albumin(ALB)],immune function indexes[immunoglobulin(Ig)G,IgM,IgA,CD3^(+)and CD4^(+)/CD8^(+)],adverse reactions and prognostic indicators[body mass index,patient-generated subjective global assessment(PG-SGA)score and European Organization for Research and Treatment of Cancer quality of life questionnaire core 30(EORTC QLQ-C30)score]of the two groups were compared.Result The clinical efficacy of the immune group was better than that of the conventional group,and the total effective rate was higher than that of the conventional group,the differences were statistically significant(P<0.05).After treatment,the levels of CRP,PCT,IL-6,TNF-α,TRF,ALB,PAB and TP in both groups were higher than those before treatment,and the levels of CRP,PCT,IL-6 and TNF-αin immune group were lower than those in conventional group,the levels of TRF,ALB,PAB and TP were higher than those in conventional group,the differences were statistically significant(P<0.05).After treatment,the levels of IgG,IgM,IgA,CD3^(+)and CD4^(+)/CD8^(+)in immune group were higher than those in conventional group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions in immune group was lower than that in conventional group(P<0.05).After treatment,the body mass indexes and EORTC QLQ-C30 scores in both groups were higher than those before treatment,the PG-SGA scores were lower than those before treatment,the body m
