摘要
目的:探究PDCA循环法对急性缺血性卒中(AIS)患者静脉溶栓的预后影响。方法:选取2022年10月—2023年7月于北京市密云区医院接受重组组织型纤溶酶原激活剂-阿替普酶(rt-PA)静脉溶栓治疗的AIS患者80例作为研究对象,随机分为常规管理组(采用常规静脉溶栓管理)和循环管理组(在常规管理组基础上采用PDCA循环法管理),各40例。比较两组治疗相关时间、美国国立卫生研究院卒中量表(NIHSS)评分、预后效果及不良事件发生率。结果:循环管理组CT完成时间、知情同意时间、入院至静脉溶栓治疗时间短于常规管理组,差异有统计学意义(P<0.001);溶栓后,两组NIHSS评分均低于溶栓前,且循环管理组低于常规管理组,差异有统计学意义(P<0.05);循环管理组改良Rankin量表评分低于常规管理组,差异有统计学意义(P<0.001);循环管理组患者不良事件发生率低于常规管理组,差异有统计学意义(P=0.040)。结论:PDCA循环法有助于缩短AIS患者rt-PA静脉溶栓治疗时间,可改善神经功能,减少不良事件的发生。
Objective:To explore the prognostic effect of PDCA circulation on intravenous thrombolysis in patients with acute ischemic stroke(AIS).Methods:A total of eighty AIS patients who received intravenous thrombolytic therapy with recombinant tissue plasminogen activator Alteplase(rt-PA)in Beijing Miyun District Hospital from October 2022 to July 2023 were selected as the study objects.Randomly divided into routine management group(routine intravenous thrombolysis management)and circulation management group(PDCA circulation management based on routine management group),forty cases each.Treatment-related time,National institutes of health stroke scale(NIHSS)score,prognostic effect and incidence of adverse events were compared between the two groups.Results:CT completion time,informed consent time,hospital admission to intravenous thrombolytic therapy time in the circulation management group were shorter than those in the routine management group,and the differences were statistically significant(P<0.001);after thrombolysis,NIHSS scores in both groups were lower than before thrombolysis,and the circulation management group was lower than the routine management group,the difference was statistically significant(P<0.05);the improved Rankin scale score of the circulation management group was lower than that of the routine management group,and the difference was statistically significant(P<0.001);the incidence of adverse events in circulation management group was lower than that in routine management group,and the difference was statistically significant(P=0.040).Conclusion:PDCA circulation is helpful to shorten the time of rt-PA intravenous thrombolytic therapy in AIS patients,improve nerve function and reduce the occurrence of adverse events.
