摘要
目的 探讨磁共振DWI与血清肿瘤标志物对妇科良恶性肿瘤的评价。方法 选取本院诊治的168例妇科肿瘤患者资料,根据穿刺或手术病理检查分为良性组92例、恶性组76例,并根据病理分期将恶性组分为Ⅰ期(32例)、Ⅱ期(18例)、Ⅲ期(15例)、Ⅳ期(11例)。对比表观扩散系数(ADC)、糖类抗原153(CA153)、糖类抗原125(CA125)、癌胚抗原(CEA)。结果 恶性组的ADC低于良性组,而CA153、CA125、CEA高于良性组,差异有统计学意义(P<0.05)。Ⅳ期患者的ADC分别低于Ⅰ期、Ⅱ期、Ⅲ期,而CA153、CA125、CEA高于Ⅰ期、Ⅱ期、Ⅲ期,差异有统计学意义(P<0.05)。受试者工作特征曲线(ROC)分析显示ADC、CA153、CA125、CEA及四项联合诊断妇科良恶性肿瘤的AUC值分别为0.842、0.811、0.848、0.772、0.909,各项预测效能比较差异有统计学意义(P<0.05)。结论 DWI联合血清肿瘤标志物鉴别诊断妇科良恶性肿瘤具有较高的效能。
Objective To explore the evaluation of magnetic resonance DWI and serum tumor markers in gynecological be‐nign and malignant tumors.Methods Data of 168 gynecological tumor patients treated in our hospital were selected.Those pa‐tients were divided into benign group(92 cases)and malignant group(76 cases)according to puncture or surgical pathological examination,and the patients in malignant group were further divided into stage I(n=32 cases),stage II(n=18 cases),stage III(n=15 cases),and stage IV(n=11 cases)according to pathological stage.The apparent diffusion coefficient(ADC),carbo‐hydrate antigen 153(CA153),carbohydrate antigen 125(CA125)and carcinoembryonic antigen(CEA)were compared.Re-sults The ADC of the malignant group was lower than that of the benign group,while,the levels of CA153,CA125,and CEA were higher than those of the benign group(P<0.05).The ADC of stage IV patients was higher than that of stage I,II,and III,respectively,while,CA153,CA125,and CEA were lower than those of stage I,II,and III(P<0.05).ROC curve analysis showed that the AUC values of ADC,CA153,CA125,CEA and the combined diagnosis of benign and malignant gynecological tumors were 0.842,0.811,0.848,0.772,0.909,respectively(P<0.05).Conclusion DWI combined with serum tumor mark‐ers is effective in differentiating benign and malignant gynecological tumors.
