EEG参数与癫痫患儿认知损伤分级的关系研究

摘要 目的探讨脑电图(EEG)参数与癫痫患儿认知损伤分级的关系。方法回顾性分析2021年1月—2023年12月于本院体检的130名健康儿童的资料,纳入对照组;选择同期于本院就诊的130例癫痫患儿资料,纳入观察组,两组疾病资料或体检资料、检查结果等资料均保存完整。整理研究对象资料,记录并对比对照组与观察组的EEG参数、记录观察组患儿的韦氏儿童智力量表第4版(WISC-Ⅳ)评估结果,并据此将患儿分为智力正常(n=34)、智力异常/临界状态(n=58)、智力损害(n=38);记录并对比不同认知损伤程度患儿的基线资料、EEG参数,重点分析EEG参数与癫痫患儿认知损伤分级的关系。结果相较于对照组,观察组的α波功率较低,θ波功率较高(P<0.05);两组的β、δ波功率比较,异无统计学意义(P>0.05)。不同认知损伤分级患儿在性别、年龄、身高等基线资料方面比较,异无统计学意义(P>0.05);三组间首次发病年龄、病程、发作频率方面比较,差异有统计学意义(P<0.05)。不同认知损伤程度患儿的β、δ波功率比较,异无统计学意义(P>0.05);三组患儿在α、θ波功率、清醒期放电指数、睡眠期放电指数方面比较,异有统计学意义(P<0.05),且随着认知损伤程度升高,患儿α波功率降低,θ波功率、清醒期放电指数、睡眠期放电指数升高。以智力正常为参照,行有序回归分析结果显示,首次发病年龄、病程、发作频率≥4次/月、α波功率(降低)、θ波功率(升高)、清醒期放电指数>10%、睡眠期放电指数1%~10%或>10%可能与癫痫患儿认知损伤程度有关(P<0.05)。结论EEG参数与癫痫患儿认知功能有密切关系,主要参数包括α波功率、θ波功率、清醒期放电指数、睡眠期放电指数,且随着α波功率降低,θ波功率、清醒期放电指数、睡眠期放电指数升高,可能提示认知损伤加重。 Objective To investigate the relationship between electroencephalogram(EEG)parameters and cognitive impairment grading in children with epilepsy.Methods A retrospective analysis was conducted.The data of 130 healthy children who underwent physical examination in the hospital from January 2021 to December 2023 were selected and included in the control group.The data of 130 children with epilepsy who were treated in the hospital during the same period were selected and included in the observation group.The disease data or physical examination data,examination results and other data of the two groups were intact.The data of the subjects were collected,the EEG parameters of the control group and the observation group were recorded and compared,and the evaluation results of the Wechsler Intelligence Scale for Children-Fourth Edition(WISC-Ⅳ)in the observation group were recorded.According to this,the children were divided into normal intelligence(n=34),abnormal intelligence/critical state(n=58),and intellectual impairment(n=38).The baseline data and EEG parameters of children with different degrees of cognitive impairment were recorded and compared,and the relationship between EEG parameters and the classification of cognitive impairment in children with epilepsy was analyzed.Results Compared with the control group,theαwave power of the observation group was lower and theθwave power was higher(P<0.05).There was no statistical significant difference inβandδwave power between the two groups(P>0.05).There were no statistical significant difference in baseline data such as gender,age and height among children with different cognitive impairment grades(P>0.05).There were statistical significant differences in the age of first onset,course of disease and frequency of attack among the three groups(P<0.05).There was no statistical significant difference inβandδwave power between children with different degrees of cognitive impairment(P>0.05).There were statistical significant differences inα,θwave power,awake dischar
