经鼻高流量氧疗在肺结核合并I型呼吸衰竭患者治疗中的应用效果研究

Study on the Application Effect of Nasal High Flow Oxygen Therapy in the Treatment of Pulmonary Tuberculosis Patients with Type I Respiratory Failure

摘要 观察经鼻高流量氧疗(high-fl ow nasal cannula oxygen therapy,HFNC)治疗肺结核合并I型呼吸衰竭的临床效果。以2022年1月—2023年12月在贵阳市公共卫生救治中心结核科住院的60例确诊为肺结核合并I型呼吸衰竭患者为研究对象,将其随机分为观察组和对照组两组,每组各30例。观察组行HFNC治疗,对照组行常规氧疗,比较两组临床治疗效果。结果显示,治疗前,两组血气指标、生理指标、氧合指数比较,无统计学差异(P>0.05);治疗后,观察组PaO_(2)、pH、SpO_(2)指标高于对照组,PaCO_(2)、HR、RR、住院时间、机械通气使用率等指标低于对照组(P<0.05)。研究发现,针对肺结核合并I型呼吸衰竭患者实施HFNC治疗,可有效改善患者的血气指标、生理指标、氧合指数,缩短其住院时间,降低机械通气使用率。 Observation of the clinical efficacy of high fl ow nasal cannula oxygen therapy(HFNC)in the treatment of pulmonary tuberculosis with type I respiratory failure.Sixty patients diagnosed with pulmonary tuberculosis and type I respiratory failure admitted to the tuberculosis department of Guiyang Public Health Clinical Center from January 2022 to December 2023 were randomly divided into an observation group and a control group,with 30 patients in each group.The observation group received HFNC treatment,while the control group received routine oxygen therapy,and the clinical treatment effects of the two groups were compared.The results showed that there was no statistically significant difference(P>0.05)in blood gas indicators,physiological indicators,and oxygenation index between the two groups before treatment;After treatment,the PaO_(2),pH,and SpO_(2) indicators in the observation group were higher than those in the control group,while PaCO_(2),HR,RR,length of hospital stay,and mechanical ventilation utilization rate were lower than those in the control group(P<0.05).Research has found that implementing HFNC treatment for patients with pulmonary tuberculosis and type I respiratory failure can effectively improve their blood gas indicators,physiological indicators,oxygenation index,shorten their hospitalization time,and reduce the use of mechanical ventilation.
