摘要
目的 探索光电容积脉搏波振幅(PPGA)用于评估手术患者术前焦虑程度的可行性。方法 依据状态-特质焦虑量表中的状态焦虑量表(STAI-S)评分将113例择期行外科手术的患者分为焦虑组(术前STAI-S评分≥40分,57例)和非焦虑组(STAI-S评分<40分,56例)。采用光电监护仪记录两组患者PPGA。采用Pearson相关系数分析STAI-S评分与各临床指标之间的相关性。ROC曲线分析PPGA对患者术前焦虑的诊断价值。结果 焦虑组PPGA低于非焦虑组(P<0.01)。两组心率、收缩压、舒张压均无统计学差异(P>0.05)。STAI-S评分与PPGA呈负相关(r=-0.43,P<0.01),与心率、收缩压、舒张压无相关性(r=0.11、0.11、0.02,P>0.05)。ROC曲线结果显示,PPGA诊断患者术前焦虑的曲线下面积为0.77[95%CI(0.68~0.86)],当取最佳诊断界值为13.5任意单位时,其对应的灵敏度和特异度分别为79.6%和71.2%。结论 PPGA可用于评估患者术前焦虑状态。患者PPGA越低,术前焦虑程度越高。
Objective To explore the feasibility of pulse photoplethysmographic amplitude(PPGA)in assessing the preoperative anxiety degree in the patients undergoing surgery.Methods According to the State-Trait Anxiety Inventory-State(STAI-S)score,1l13 patients undergoing selective surgery were divided into two groups of A(preoperative STAI-S score≥40 points,57 cases)and B(preoperative STAI-S score<40 points,56 cases).The PPGA of the patients in two groups was recorded using a photoelectric monitor.Pearson correlation coefficient was used to analyze the correlation between STAI-S score and other indicators.ROC curve was used to analyze the diagnostic value of PPGA for the preoperative anxiety of the patients.Results The PPGA of group A was lower than that of group B(P<0.01).There were no significant differences in heart rate,systolic blood pressure and diastolic blood pressure between the two groups(P>0.05).STAI-S score was negatively correlated with PPGA(r=-0.43,P<0.01),but not with heart rate,systolic blood pressure and diastolic blood pressure(r=0.11,0.11 and 0.02,P>0.05).The ROC curve results showed that the AUC of PPGA in evaluating preoperative anxiety was 0.77[95%CI(0.68-0.86)].When the optimal cut-off value was 13.5 arbitrary units,the sensitivity and specificity were 79.6%and 71.2%,respectively.Conclusion PPGA can be used to evaluate the preoperative anxiety status of patients.The lower the PPGA,the higher the preoperative anxiety degree of the patient.
