摘要
目的研究运动想象训练联合优化运动技能训练对老年脑卒中后上肢功能障碍患者上肢肌肉表面肌电信号(sEMG)指标、肘关节屈伸功能、上肢和手功能的影响。方法选取开封市中医院2021年10月至2023年10月107例脑卒中后上肢功能障碍患者作为研究对象,采用随机数字表分组,对照组53例给予运动想象训练,研究组54例在对照组基础上联合优化运动技能训练,对比两组患者上肢肌肉sEMG指标、肘关节屈伸功能、上肢和手功能。结果研究组干预后,肱三头肌、三角肌、腕伸肌、肱二头肌的均方根(RMS)水平分别为(31.32±3.45)μV、(32.34±3.58)μV、(26.34±4.16)μV、(32.48±3.54)μV,均高于对照组(24.77±3.23)μV、(27.65±3.64)μV、(18.45±4.43)μV、(27.64±3.88)μV,肱三头肌、三角肌、腕伸肌、肱二头肌的中位频率(MF)水平分别为(74.45±5.66)Hz、(87.58±5.42)Hz、(50.37±4.88)Hz、(89.57±4.24)Hz,均高于对照组(65.75±5.34)Hz、(81.24±4.88)Hz、(44.76±4.37)Hz、(83.45±4.74)Hz(P<0.05);研究组干预后,Fugl-Meyer上肢评定量表(FMA-UE)、布朗茨手部测试量表(BzH)评分分别为(45.32±3.74)分、(42.47±3.42)分,均高于对照组(38.55±4.14)分、(31.43±3.22)分(P<0.05);研究组干预后,肘关节屈协同收缩率、肘关节伸协同收缩率分别为(14.84±2.31)%、(31.44±4.12)%,均低于对照组(19.11±2.44)%、(42.77±4.43)%(P<0.05)。结论优化运动技能训练联合运动想象训练对脑卒中后上肢功能障碍患者效果显著,能够有效提高肘关节屈伸功能、上肢和手功能。
Objective To investigate the effects of motor imagination training combined with optimized motor skill training on the surface electromyographic(sEMG)indicators,of the upper limbs,elbow flexion and extension function,and upper limb and hand function in elderly patients with upper limb dysfunction after stroke.Methods A total of 107 patients with post-stroke upper limb dysfunction in Kaifeng Hospital of TCM from October 2021 to October 2023 were recruited.They were randomly assigned into the control group(n=53,motor imagination training)and research group(n=54,motor imagination training combined with optimized motor skill training)by a random number table.sEMG indicators of upper limb muscle,elbow flexion and extension function,and upper limb and hand function were compared between groups.Results After intervention,the root mean square(RMS)of triceps([31.32±3.45]μV VS[24.77±3.23]μV),deltoid([32.34±3.58]μV VS[27.65±3.64]μV),wrist extensor([26.34±4.16]μV VS[18.45±4.43]μV)and biceps([32.48±3.54]μV VS[27.64±3.88]μV)was significantly higher in the research group than in the control group.The median frequency(MF)of triceps([74.45±5.66]Hz VS[65.75±5.34]Hz),deltoid([87.58±5.42]Hz VS[81.24±4.88]Hz),extensor([50.37±4.88]Hz VS[44.76±4.37]Hz)and biceps([89.57±4.24]Hz VS[83.45±4.74]Hz)was significantly higher in the research group than in the control group(P<0.05).After intervention,the scores of Fugl-Meyer assessment-upper extremity(FMA-UE,[45.32±3.74]points VS[38.55±4.14]points)and Blounz hand testing scale(BzH,[42.47±3.42]points VS[31.43±3.22]points)were significantly higher in the research group than in the control group(P<0.05).After intervention,the co-contraction rates of elbow flexion([14.84±2.31]%VS[19.11±2.44]%)and extension([31.44±4.12]%VS[42.77±4.43]%]were significantly lower in the research group than in the control group(P<0.05).Conclusion Optimized motor skill training combined with motor imagination training has a significant effect on patients with post-stroke upper limb dysfunc
