摘要
目的探讨低温等离子消融术与腺样体刮除术及双侧扁桃体剥离术治疗儿童阻塞性睡眠呼吸暂停低通气综合征的临床疗效。方法选取2019年1月至2021年6月吉安市中心人民医院收治的88例需进行手术治疗的阻塞性睡眠呼吸暂停低通气综合征患儿作为研究对象,随机分为观察组与对照组,每组44例。观察组行低温等离子消融术治疗,对照组行腺样体刮除术联合双侧扁桃体剥离术治疗。比较两组围手术期指标、临床疗效、术后疼痛评分[疼痛数字分级法(NRS)]及不良反应发生情况。结果观察组手术时间短于对照组,手术出血量少于对照组,术后疼痛时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率为95.45%,高于对照组的79.55%,差异有统计学意义(P<0.05)。术后6、24、48、72h,观察组NRS评分均低于对照组,差异有统计学意义(P<0.05)。两组手术中均未出现出血过量、术中疼痛及血压升高等不良反应。结论与传统的腺样体刮除术联合双侧扁桃体剥离术相比,低温等离子消融术具有良好的可行性,手术安全有效,值得临床推广应用。
Objective To investigate the clinical efficacy of low temperature plasma ablation and adenoid curettage combined with bilateral ton-sillectomy in the treatment of obstructive sleep apnea hypopnea syndrome in children.Methods A total of 88 children with obstructive sleep apnea hypopnea syndrome requiring surgical treatment in Ji'an Central People's Hospital from January 2019 to June 2021 were selected as the research sub-jects,and they were randomly divided into the observation group and the control group,with 44 cases in each group.The observation group was treat-ed with low temperature plasma ablation,the control group was treated with adenoid curettage combined with bilateral tonsillectomy.The periopera-tive indexes,clinical efficacy,postoperative pain score(numerical rating scale[NRS])and adverse reactions were compared between the two groups.Results The operation time in the observation group was shorter than that in the control group,the amount of bleeding was less than that in the control group,and the postoperative pain time and hospitalization time were shorter than those in the control group,the differences were statistically signifi-cant(P<0.05).The total effective rate of treatment in the observation group was 95.45%,which was higher than 79.55%in the control group,and the difference was statistically significant(P<0.05).At 6,24,48 and 72 h after operation,the NRS scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Compared with the traditional adenoid curet-tage combined with bilateral tonsillectomy,low temperature plasma ablation is feasible,safe and effective,which is worthy of clinical promotion and application.
