摘要
皮瓣外科技术是显微外科医师常用的外科技术,也是软组织缺损后创面覆盖的主要技术手段。20世纪70年代以来,随着对显微外科小血管吻合技术和穿支血管解剖的深入认识,游离皮瓣和穿支皮瓣更新了组织移植的传统观念,极大地推动了皮瓣外科的发展。游离皮瓣解决了过去处理极为棘手、治疗效果不佳甚至无法修复的创面难题,是皮瓣外科发展史上重要的里程碑。穿支皮瓣突破了“深筋膜血管网是皮瓣赖以生存的必备条件”的传统观念,使皮瓣移植迈向了“自由王国”,实现了以最小的供区损害获得最佳的受区外形和功能,代表目前皮瓣外科的最新进展,是皮瓣外科发展史上又一次飞跃。
Microvascular free flap reconstruction was a routine option for the coverage of a variety of tissue defects.The development of perforator flaps has greatly benefited from recent advances in imaging techniques.Preoperative mapping reduced both procedure duration and morbidity.The digital technology for preoperative design can achieve precise and personalized soft tissue repair.Virtual reality was one of the fast-developing technologies that allow the three-dimensional visualization of digital information,and this technology has the potential to assist the plastic surgeon in operating more accurately,safely,and fast.Accurate monitoring of laser speckle and laser Doppler perfusion imaging in the postoperative period was a crucial part of successful flap surgery allowing early detection of vascular compromise and prompt intervention in flap salvaging.The new robot designed specifically for microsurgery may reduce human tremor and thus enhance precision.Intelligence and digitalization are the necessary trend of flap surgery in the coming future.
