抗结核药物引起儿童肝损伤的危险因素分析

摘要 目的研究抗结核药物引起儿童肝损伤(ATDILI)的危险因素。方法选取2021年1月至2022年12月在泰安市肿瘤防治院住院的确诊为结核病并行抗结核治疗的100例患儿为研究对象,根据药物不良反应相关指南确诊ATDILI的31例患儿为观察组,未发生ATDILI的69例患儿为对照组。采用logistic回归分析ATDILI的危险因素。结果两组年龄、治疗情况、合并肺外结核、贫血、低蛋白血症、营养不良比较,差异有统计学意义(P<0.05)。logistic回归分析结果显示,年龄、治疗情况、合并肺外结核、贫血、低蛋白血症、营养不良是ATDILI的独立危险因素(P<0.05)。结论年龄、治疗情况、合并肺外结核、贫血、低蛋白血症、营养不良是影响ATDILI的独立危险因素。 Objective To study the risk factors of anti-tuberculosis drugs-induced liver injury(ATDILI)in children.Methods A total of 100 children admitted to Taian Cancer Prevention and Treatment Hospital from January 2021 to December 2022 who were diagnosed with tuberculosis and treated with anti-tuberculosis therapy were selected as the study objects,31 children diagnosed with ATDILI according to guidelines related to adverse drug reactions were selected as the observation group,and 69 children without ATDILI were selected as the control group.The risk factors of ATDILI were analyzed by logistic regression.Results There were statistically significant differences in age,treatment,combined extra-pulmonary tuberculosis,anemia,hypoproteinemia,and malnutrition between the two groups(P<0.05).The logistic regression analysis showed that age,treatment,combined with extra-pulmonary tuberculosis,anemia,hypoproteinemia,and malnutrition were independent risk factors for ATDILI(P<0.05).Conclusion Age,treatment status,concurrent extrapulmonary tuberculosis,anemia,hypoalbuminemia,and malnutrition are independent risk factors affecting ATDILI.
