摘要
目的:评估中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、单核细胞与淋巴细胞比值(MLR)对老年患者肺部感染的辅助诊断价值。方法:回顾性分析54例老年肺部感染患者及54例健康体检的老年人血常规,并计算NLR、PLR、MLR;多因素Logistic回归分析老年人肺部感染的相关危险因素;ROC曲线预测相关指标对患者的诊断价值。结果:老年肺部感染患者NLR、PLR、MLR水平均高于对照组(P<0.05)。多因素Logistic回归模型及矫正模型均显示NLR升高是老年肺部感染的独立危险因素(P<0.01)。NLR预测老年肺部感染的ROC曲线下面积为0.840。结论:NLR是老年肺部感染良好的辅助诊断指标,具有较高的临床实用价值。
Objective:To assess the value of neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR)and monocyte-to-lymphocyte ratio(MLR)in auxiliary diagnosis of pulmonary infections in elderly patients.Methods:The routine blood test results were retrospectively analyzed in 54 elderly patients with pulmonary infections and 54 healthy controls,and NLR,PLR and MLR were calculated,respectively.Multivariate logistic regression model was used to analyze the risk factors for pulmonary infection in the elderly patients,and ROC curve was used to estimate the diagnostic value of the three indicators.Results:The levels of NLR,PLR and MLR in elderly patients with pulmonary infection were higher than those in control group(P<0.05).Multivariate logistic regression analysis showed that elevated NLR was an independent risk factor for the infections.The areas under ROC curve was 0.840 for NLR.Conclusion:NLR is of higher clinical implications,because it is a good auxiliary diagnostic indicator for estimating pulmonary infection in elderly patients.
