摘要
目的探讨2型糖尿病人群胰岛素早相分泌功能的影响因素,指导临床改善胰岛功能。方法选取2型糖尿病患者137名,行葡萄糖耐量试验测定多点血糖及C肽。根据早相胰岛素分泌指数(early insulin secretion,IGI)水平分为低IGI组、中IGI组、高IGI组,行单因素及多因素Logistic回归分析。结果高IGI组体质量指数、空腹C肽、尿酸、甘油三脂、稳态模型β细胞分泌指数、脂肪肝高于低IGI组(P<0.05),高IGI组120min葡萄糖、糖化血红蛋白低于低IGI组(P<0.05)。中IGI组体质量指数、空腹C肽、脂肪肝高于低IGI组(P<0.05),中IGI组120min葡萄糖、糖化血红蛋白高于低IGI组(P<0.05)。多元Logistic回归分析结果显示糖化血红蛋白(OR=0.796,95%CI:0.676~0.938,P<0.05)是IGI30的独立危险因素,而稳态模型胰岛素抵抗指数(OR=3.161,95%CI:1.324~7.553,P<0.05)、甘油三酯(OR=1.287,95%CI:1.005~1.650,P<0.05)是IGI30的保护性危险因素。结论糖化血红蛋白是2型糖尿病早相胰岛功能分泌的危险因素,胰岛素抵抗是早相胰岛素功能分泌的保护性因素。
Objective To investigate the factors affecting the early phase insulin secretion function in type 2 diabetes patients and guide the clinical improvement of islet function.Methods A total of 137 patients with type 2 diabetes mellitus were selected to undergo glucose tolerance test to determine multipoint blood glucose and C-peptide.According to the level of early insulin secretion index(IGI),secretion is divided into low IGI group,medium IGI group and high IGI group.Univariate and multivariate Logistic regression analysis was performed.Results Body mass index,fasting C-peptide,uric acid,triglyceride,steady-state modelβcell secretion index and fatty liver in high IGI group were higher than those in low IGI group(P<0.05).The 120-minute glucose and HBA1c in high IGI group were lower than those in low IGI group(P<0.05).Body mass index,fasting C-peptide and fatty liver in medium IGI group were higher than those in low IGI group(P<0.05).The 120-minute glucose and HBA1c in medium IGI group were higher than those in low IGI group(P<0.05).Multivariate Logistic regression showed that glycosylated hemoglobin(OR=0.796,95%CI:0.676-0.938,P<0.05)was an independent risk factor for IGI30,and the steady-state insulin resistance index(OR=3.161,95%CI:1.324-7.553,P<0.05),triglyceride(OR=1.287,95%CI:1.005-1.650,P<0.05)is a protective risk factor for IGI30.Conclusion Glycosylated hemoglobin is a risk factor for the secretion of pancreatic islet in early stage of type 2 diabetes mellitus.Insulin resistance is a protective factor for the secretion of early phase insulin function.
