摘要
目的分析免疫球蛋白治疗儿童脓毒症对免疫功能的影响与疗效。方法简单随机选取淄博市妇幼保健院于2022年1月—2023年6月收治的80例脓毒症患儿为研究对象。按照随机抽签法分为对照组、观察组,每组40例。对照组应用甲泼尼龙常规治疗,观察组采用常规疗法+免疫球蛋白。比较两组治疗有效性、免疫功能、炎性因子指标与治疗安全性。结果观察组治疗总有效率为95.00%,高于对照组的80.00%,差异有统计学意义(χ^(2)=4.114,P<0.05)。治疗前,两组T淋巴细胞亚群及炎性指标比较,差异无统计学意义(P均>0.05),治疗后,观察组CD4^(+)、CD4^(+)/CD8^(+)均高于对照组,CD8^(+)、白介素-6与肿瘤坏死因子-α低于对照组,差异有统计学意义(P均<0.05)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论对脓毒症患儿在常规治疗的基础上增加免疫球蛋白治疗能够提高治疗有效性,发挥免疫调节机制,改善患儿免疫功能状态,减轻炎症反应,不良反应少,药物安全性良好。
Objective To analyze the effect of immunoglobulin on immune function in the treatment of sepsis in children.Methods A total of eighty children with sepsis admitted to Zibo Maternal and Child Health Hospital from January 2022 to June 2023 were simple randomly selected as the study objects.They were divided into control group and observation group according to random drawing,with forty cases in each group.The control group received conventional treatment with methylprednisolone,and the observation group received conventional therapy + immunoglobulin.The therapeutic efficacy,immune function,inflammatory factor indexes and therapeutic safety were compared between the two groups.Results The total effective rate of the observation group was 95.00% higher than that of the control group(80.00%),and the difference was statistically significant(χ^(2)=4.14,P<0.05).There was no significant difference in T lymphocyte subsets and inflammatory indexes between the two groups before treatment(both P>0.05),after treatment,CD4^(+) and CD4^(+)/CD8^(+) in observation group were higher than control group,CD8^(+),interleukin-6 and tumor necrosis factor-α were lower than control group,the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion The addition of immunoglobulin therapy on the basis of conventional treatment for children with sepsis can improve the effectiveness of treatment,exert the immune regulatory mechanism,improve the immune function of children,reduce inflammation,less adverse reactions,and have good drug safety.
