摘要
目的:探讨加速康复外科理念护理对人工股骨头置换术患者术后疼痛和康复的干预效果。方法:选取2021年1月-2023年1月期间在我院进行人工股骨头置换术的80例患者作为研究对象,按照随机数字法将其分为观察组和对照组,每组均纳入40例患者。对照组患者采取围术期常规护理方案,观察组患者则采取加速康复外科(ERAS)护理方案,对2组患者术后疼痛程度和术后康复情况进行对比。结果:2组患者在返回病房时的VAS评分比较,差异无统计学意义(P>0.05);但是在术后第1天清晨、术后第2天清晨和术后第3天清晨观察组患者的评分要明显低于对照组,组间差异有统计学意义(P<0.05);观察组患者的首次下床活动时间、首次下床活动距离、首次排气时间、首次排便时间、尿管拔出时间和引流管拔出时间均明显短于对照组,组间差异有统计学意义(P<0.05);出院前,观察组患者出院前和术后1个月的Harris髋关节功能评分和Barthel指数评分均高于对照组,组间差异有统计学意义(P<0.05);观察组患者术后并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。结论:加速康复外科理念护理可有效减轻人工股骨头置换术患者术后疼痛程度,促进髋关节功能的恢复,提升患者术后的日常生活能力。
Objective:To explore the effect of enhanced recovery after surgery nursing on postoperative pain and rehabilitation of patients with artificial femoral head replacement.Methods:A total of 80 patients who underwent artificial femoral head replacement in our hospital from January 2021 to January 2023 were selected as the research objects.According to the random number method,they were divided into an observation group and a control group,with 40 patients in each group.Patients in the control group received routine perioperative nursing,while patients in the observation group received Enhanced Recovery After Surgery(ERAS)nursing.The degree of postoperative pain and postoperative rehabilitation of the two groups were compared.Results:There was no statistically significant difference in VAS score between the two groups when returning to the ward(P>0.05).However,the scores of the observation group were significantly lower than those of the control group in the morning of the first day,the second day and the third day after the operation,and the differences between the two groups were statistically significant(P<0.05).The time to get out of bed for the first time,the distance to get out of bed for the first time,the time to exhaust for the first time,the time to defecate for the first time,the time to remove the urinary catheter and the time to remove the drainage tube in the observation group were significantly lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).Before discharge,the Harris hip function score and Barthel index score of the observation group before discharge and one month after operation were higher than those of the control group,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:ERAS nursing can effectively reduce the postoperative pain of patients with artificial femoral head replaceme
