摘要
目的观察和评价基于超声分型及定量评分指导剖宫产瘢痕妊娠(CSP)不同手术方式的安全性及有效性。方法以2021年7月-2023年7月期间于九江学院附属医院就诊的剖宫产瘢痕妊娠患者100例为研究对象,根据手术方式将患者分为宫腔镜组和宫腔镜+子宫动脉栓塞术(UAE)组,回顾性收集患者术前一般资料、围手术期资料,基于术前超声结果对CSP进行分型判断和超声量化评分,对上述指标和资料进行统计学分析。结果宫腔镜组和宫腔镜+UAE组CSP分型分布之间差异具有统计学意义(P<0.05),以超声分型预测是否需要UAE处理的受试者工作曲线(ROC)的曲线下面积为0.697,敏感度为83.3%,特异性为51.7%;宫腔镜组超声定量评分低于宫腔镜+UAE组,差异具有统计学意义(P<0.05),以超声定量评分预测是否需要UAE处理曲线下面积为0.773,敏感度为50.0%,特异性为91.4%,Cut-off值为4.785。超声分型和定量评分联合预测的曲线下面积为0.825,敏感度为78.6%,特异性为81.0%。结论基于超声分型及定量评分选择剖宫产瘢痕妊娠手术方式具有一定的使用意义,较为安全。
Objective To observe and evaluate the safety and efficacy of different surgical methods for cesarean scar pregnancy(CSP)based on ultrasound classification and quantitative scoring.Methods A total of 100 patients with cesarean scar pregnancy in the Affiliated Hospital of Jiujiang University from July 2021 to July 2023 were selected as the research objects.According to the surgical method,the patients were divided into hysteroscopy group and hysteroscopy+uterine artery embolization(UAE)group.CSP was classified and scored based on the preoperative ultrasound results,and the above indicators and data were statistically analyzed.Results There were significant differences in CSP types between hysteroscopy group and hysteroscopy+UAE group(P<0.05);the area under the receiver operating characteristic(ROC)curve was 0.697,the sensitivity was 83.3%and the specificity was 51.7%.The ultrasound quantitative score of hysteroscopy group was lower than that of hysteroscopy+UAE group,and the difference was statistically significant(P<0.05);the area under the curve of ultrasound quantitative score for predicting UAE was 0.773,the sensitivity was 50.0%,the specificity was 91.4%,and the Cut-off value was 4.785.The area under the curve of the combination of ultrasound classification and quantitative score was 0.825,the sensitivity was 78.6%,and the specificity was 81.0%.Conclusions Based on ultrasound classification and quantitative score,the choice of surgical methods for cesarean scar pregnancy has certain application significance and is safe.
