摘要
目的探讨帕金森病患者及其照护者对居家康复锻炼的真实体验,深入挖掘居家康复的影响因素,为提高帕金森病患者居家康复质量提供参考依据。方法采用描述性质性研究,选取16对居家康复锻炼的帕金森病患者及其主要照护者进行半结构式访谈,基于Bronfenbrenner生态系统理论模型、利用主题分析法和配对分析法对访谈资料进行分析和提炼主题。结果提炼出微系统——帕金森病患者个体特质(疾病现状与进展、心理弹性与应对方式、创伤后成长、运动自我效能与动机)、中系统——家庭抗逆力系统(主要照护者微系统特质、家庭信念系统、家庭组织模式、家庭沟通过程)、外系统——相关社会领域及宏系统——文化环境4个主题。结论帕金森病患者居家康复运动受多种因素的影响,患者及其家庭应充分激发个体主观能动性、重视主要照护者照护功能、调动家庭抗逆力实现家庭复原、合理利用社会资源,医疗教育等社会领域应充分聚焦专科性发展、建立支持系统协同实现帕金森病患者居家康复。
Objective To explore the real experience of home rehabilitation exercise in Parkinson′s disease patients and their caregivers,and to deeply dig the influencing factors of home rehabilitation,so as to provide a reference for improving home rehabilitation quality of patients with Parkinson′s disease.Methods This is a descriptive qualitative study.A total of 16 pairs of Parkinson′s disease patients who implemented rehabilitation exercise at home and their primary caregivers were selected for semi-structured interviews.Based on the Bronfenbrenner′s ecological systems theory,the thematic analysis and dyadic qualitative analysis approach were used to analyze the interview data and extract themes.Results Four themes were extracted.Micro-systems:individual characteristics of Parkinson′s disease patients(disease status and progression,resilience and coping style,post-traumatic growth,exercise self-efficacy and motivation).Meso-systems:family resilience system(primary caregivers′micro-system characteristics,family belief system,family organization model,family communication process).Exo-systems:related social fields.Macro-systems:cultural environment.Conclusion The home rehabilitation exercise of patients with Parkinson′s disease is affected by multiple factors.Patients and their families should fully stimulate individual subjective initiative,pay attention to the care function of primary caregivers,mobilize family resistance to achieve family recovery,and make rational use of social resources.Medical education and other social fields should fully focus on specialized development and establish support systems to achieve home rehabilitation of patients with Parkinson′s disease.
