摘要
目的探究2型糖尿病(type 2 diabetes mellitus,T2DM)合并胆道结石患者行腹腔镜胆囊切除术后应用早期胰岛素强化治疗的效果。方法回顾性选取2023年8月—2024年2月在泉州市第一医院接受腹腔镜胆囊切除术的110例T2DM合并胆道结石患者的临床资料,根据不同降糖方式分为对照组(n=55,口服降糖药)和观察组(n=55,在对照组治疗基础上加入早期胰岛素强化治疗),比较两组不良反应、血糖水平、临床疗效。结果两组不良反应发生率比较,差异无统计学意义(P>0.05)。治疗4周后,观察组各项血糖水平均低于对照组,差异有统计学意义(P均<0.05)。观察组临床疗效高于对照组,差异有统计学意义(P<0.05)。结论T2DM合并胆道结石患者接受腹腔镜胆囊切除术后选择早期胰岛素强化治疗,未见不良反应明显增加,糖代谢异常得到有效纠正,临床疗效提高。
Objective To investigate the effect of early insulin intensive therapy after laparoscopic cholecystectomy in patients with type 2 diabetes mellitus(T2DM)complicated with biliary calculus.Methods Clinical data of 110 T2DM patients with biliary calculus who received laparoscopic cholecystectomy in Quanzhou First Hospital from August 2023 to February 2024 were retrospectively selected,and divided into control group(n=55,oral hypoglycemic agents)and observation group(n=55,early insulin intensive treatment was added to the treatment on the basis of of control group)according to different hypoglycemic methods.The adverse reactions,blood glucose level and clinical efficacy of the two groups were compared.Results There was no significant difference in the incidence of adverse reactions be-tween groups(P>0.05).After 4 weeks of treatment,the blood glucose levels in the observation group were lower than the control group,and the differences were statistically significant(all P<0.05).The clinical efficacy of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion After laparoscopic cholecystectomy for T2DM patients with biliary calculus,early intensive insulin therapy was selected,and no significant increase in adverse reactions was observed,abnormal glucose metabolism was effectively corrected,and clinical efficacy was improved.
