红外热成像下腰椎间盘突出症患者腰椎夹脊穴皮肤温度的研究

摘要 目的基于红外热成像技术,研究不同证型腰椎间盘突出症患者腰椎夹脊穴皮肤温度变化规律,分析二者的相关性,为针灸治疗选穴提供一定的参考。方法选取2021年1月—2021年10月的腰椎间盘突出症患者90例(寒湿型30例、血瘀型30例、肾虚型30例),另招募健康志愿者30例。应用红外热成像技术,记录寒湿型、血瘀型、肾虚型腰椎间盘突出症患者及健康者相关夹脊穴1 cm范围内的皮肤温度。结果双侧L_(1)~L_(5)夹脊穴中,健康组5个穴位与寒湿型组、瘀血型组、肾虚型组患者皮温比较,差异均有统计学意义(P<0.05)。健康组双侧L_(2)~L_(5)夹脊穴皮温与肾虚型组、寒湿型组患者比较,差异有统计学意义(P<0.05)。健康组双侧L_(5)夹脊穴皮温与寒湿型组患者比较,差异有统计学意义(P<0.05)。结论基于红外热成像技术观察不同证型腰椎间盘突出症腰椎夹脊穴皮肤温度,对诊断腰椎间盘突出有一定的辅助作用,并能为中医辨证治疗提供一定的客观依据。 Objective To analyze skin temperature variation of Jiaji(EX-B2)in lumbar intervertebral disc herniation patients with dif-ferent TCM syndromes using infrared thermography and to analyze the correlation between temperature changes and different types of lumbar disc herniation,so as to provide some reference for acupuncture and moxibustion treatment.Methods A total of 90 patients were selected with lumbar disc herniation(including cold dampness type of 30 cases,blood stasis type of 30 cases,and kidney defi-ciency type of 30 cases)from January 2021 to October 2021,and 30 healthy volunteers were recruited.Through infrared thermal imag-ing technology,the skin temperature within 1cm range of Jiaji(EX-B2)related to the cold dampness type,blood stasis type,and kid-ney deficiency type and healthy type in patients with lumbar disc herniation was recorded.Results In five points of the bilateral L_(1)-L_(5) Jiaji(EX-B2),the difference of mesothelial temperature difference distance between the healthy group and the cold dampness,blood stasis and kidney deficiency group was statistically significant(P<0.05).There was significant difference between the healthy group of bilateral L_(2)-L_(5) Jiaji(EX-B2),the kidney deficiency group and the cold dampness group(P<0.05).There was a statistically significant difference between the healthy group and the cold dampness group at L_(5) Jiaji(EX-B2)on both sides(P<0.05).Conclusion The infra-red thermal imaging temperature of Jiaji(EX-B2)in healthy people is significantly different from that of patients with lumbar disc her-niation,and the infrared thermal imaging temperature of Jiaji(EX-B2)in patients with lumbar disc herniation of different syndrome types is also significantly different,which provides a certain reference basis for the diagnosis of lumbar disc herniation and the treat-ment of lumbar disc herniation of different syndrome types with acupuncture at Jiaji(EX-B2).
