摘要
目的分析日照市单采血小板相关的输血反应,为临床预防发生单采血小板输血反应提供一定的依据。方法回顾性分析2019年1月至2022年1月日照市9家医院输单采血小板的资料,统计并分析输血反应发生的情况。结果9家医院单采血小板84065袋,67例发生输血反应,输血反应发生率为0.08%。67例输血反应中,按反应类型,最多的为过敏反应(68.66%),其次为非溶血性发热反应(20.90%);按疾病类型,最多的为急性淋巴细胞白血病(16.42%),其次为再生障碍性贫血(12.86%)以及急性髓系白血病(11.94%)。过敏反应的发生与血小板采集设备之间无关(P=0.960、0.327、0.499);而非溶血性发热反应与血小板采集设备有关,便携式血细胞采集仪MCS+发生率最高(0.038%);Amicus机器采集和Trima机器采集比较无显著差异(P=0.752);Amicus与MCS+比较有显著差异(P=0.011),Trima与MCS+比较也有显著差异(P=0.008)。结论建议医院严格按照输注血小板的指征,并提前采取预防措施,尽可能减少单采血小板输血反应的发生。
Objective To analyze the apheresis platelet transfusion reactions in Rizhao City,and to provide some guidance for the clinical prevention of apheresis platelet transfusion reactions.Methods The data of platelet apheresis transfusion in 9 hospitals in Rizhao City from Jan 2019 to Jan 202 were retrospectively analyzed,and the occurrence of transfusion reactions was counted and analyzed.Results A total of 84,065 bags of platelets were apheresed in nine hospitals,and 67 patients experienced transfusion reactions,with a transfusion reaction incidence of 0.08%.Based on reaction type,among the 67 cases of transfusion reactions,allergic reactions(68.66%)were the most common one,followed by non-hemolytic febrile reactions(20.90%).Based on illness type,the most common one is acute lymphoblastic leukemia(16.42%),followed by aplastic anemia(12.86%)and acute myeloid leukemia(11.94%).The occurrence of allergic reactions was not related to platelet collection devices(P=0.960,0.327,0.499).Non-hemolytic febrile reactions were related to platelet collection devices,while the incidence of MCS+had the highest reaction rate(0.038%),there was no significant difference between Amicus and Trima(P=0.752),there was a significant difference between Amicus and MCS+(P=0.011),and there was a significant difference between Trima and MCS+(P=0.008).Conclusion It is recommended that hospitals strictly follow the indications for platelet transfusion and take preventive measures in advance to reduce the occurrence of platelet transfusion reaction as much as possible.
