摘要
目的探讨早期康复护理对ICU重症肺炎机械通气患者预后情况的影响。方法选取2022年7月至2023年6月期间在我院ICU接受机械通气治疗的78例重症肺炎患者,采用随机数字表法分入观察组(n=40)和对照组(n=38),对照组的护理方法为常规康复护理,观察组采用早期康复护理。比较两组预后情况(ICU入住时间、机械通气时间、住院时间)、脉搏血氧饱和度(SpO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉氧分压(PaO_(2))指标及不良心理、生活质量改善情况。结果观察组机械通气患者ICU入住时间、机械通气时间、住院时间均短于对照组(P<0.05);两组干预前SDS、SAS评分、氧合指标以及QLQ-C30评分水平相比无明显差异(P>0.05),观察组干预后SDS、SAS评分、SpO_(2)、PaCO_(2)、PaO_(2)以及QLQ-C30评分水平改善显著优于对照组(P<0.05)。结论早期康复护理能有效提高ICU重症肺炎机械通气患者的预后,改善氧合指标和患者不良心理,提高患者生活质量。
Objective To investigate the effect of early rehabilitation nursing on the prognosis of ICU patients with severe pneumonia induced by mechanical ventilation.Methods A total of 78 patients with severe pneumonia who received mechanical ventilation treatment in ICU of our hospital from July 2022 to June 2023 were selected and divided into observation group(n=40)and control group(n=38)by random number table method.The control group received routine rehabilitation nursing,and the observation group received early rehabilitation nursing.The prognosis of the two groups(ICU stay time,mechanical ventilation time,hospital stay),pulse oxygen saturation(SpO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2)),poor psychology and improvement of quality of life were compared between the two groups.Results The ICU stay time,mechanical ventilation time and hospital stay of mechanical ventilation patients in the study group were shorter than those in the control group(P<0.05).There were no significant differences in SDS,SAS and other adverse psychological scores,oxygenation indexes and QLQ-C30 scores between the two groups before intervention(P>0.05).After intervention,the improvement of SDS,SAS,SpO_(2),PaCO_(2),PaO_(2) and QLQ-C30 scores in the study group was significantly better than that in the reference group(P<0.05).Conclusions Early rehabilitation nursing can effectively improve the prognosis of ICU patients with severe pneumonia mechanical ventilation,improve the oxygenation index and poor psychology of patients,and improve the quality of life of patients.
