摘要
股骨头骨髓水肿综合征属于中医学“髋痹”范畴,是一种骨科常见的自限性疾病,导致髋部疼痛及关节活动功能障碍等症状,严重危害患者日常生活质量,预后不良甚至可发生股骨头坏死等致残性疾病。肥胖患者常伴下肢负重关节的骨髓水肿情况,提示两种疾病存在某种关联,并且临床和实验研究证明肥胖症会对骨代谢产生不良影响。历代医家提出痰邪闭阻经脉骨髓可出现肢体疼痛,现代研究指出痰邪亦属肥胖症的病理产物之一,推断在肥胖伴股骨头骨髓水肿综合征的患者髋关节疼痛症状可能与痰相关。文章从痰致病的角度探讨股骨头骨髓水肿综合征的病因病机,并总结分析肥胖症影响骨代谢的最新研究成果及探讨中医脏腑病变机制,以期为今后防治股骨头骨髓水肿综合征提供新思路。
Femoral head bone marrow edema syndrome belongs to the category of traditional Chinese medicine"hip arthralgia",is a common self-limiting disease in orthopedics.It causes symptoms such as hip pain and joint mobility dysfunction,seriously endangers the patients'daily life quality,and even causes disabling diseases such as femoral head necrosis.Clinical findings indicate that obese patients are often accompanied by bone marrow edema in weight-bearing joints of lower limbs,suggesting a certain correlation between the two diseases.Moreover,clinical and experimental studies have proved that obesity has adverse effects on bone metabolism.Traditional doctors have suggested that the obstruction of meridians and bone marrow by pathogenic phlegm may lead to limb pain,but modern studies have pointed out that pathogenic phlegm is also one of the pathological products of obesity,and concluded that the symptoms of hip pain in obese patients with femoral head and bone marrow edema syndrome may be related to sputum.This paper will discuss the etiology and pathogenesis of femoral head marrow edema syndrome from the perspective of sputum pathogenesis,and summarize and analyze the latest research results of obesity affecting bone metabolism and explore the mechanism of viscera disease in traditional Chinese medicine,in order to provide new ideas for the prevention and treatment of femoral head marrow edema syndrome in the future.
