摘要
目的研究比较基线检验指标及血小板/淋巴细胞(PLR)与新型冠状病毒感染肺炎(COVID-19)中老年患者院内住院死亡的关联关系。方法调查2022年12月至2023年2月在贵阳某医院209例COVID-19确诊中老年患者。分为死亡组(n=26)、生存组(n=183)。通过回顾性方式获取数据,分析检验指标以PLR与患者死亡之间的关系。结果选取的209例患者,其中男114例(60.6%)。患者平均年龄78.02±12.59岁。多因素线性回归结果显示,在调整潜在混杂因素后,PLR与院内死亡有独立相关[HR=1.18(1.02,1.38),P=0.0303],与PLR(四分位数)作为分类变量的结果一致(P=0.038)。曲线拟合分析PLR与院内死亡之间呈非线性关系,拐点值为540。拐点左侧和右侧的HR和置信区间分别为2.0(1.4,2.9)和0.9(0.7,1.1)。结论PLR升高与病死率非线性关系,当PLR<540时,与住院死亡呈正相关。
Objective To investigate the association between comparative baseline tests and platelet/lymphocyte(PLR)and in the hospital mortality in middle-aged and elderly patients with novel coronavirus(COVID-19)infection.Methods Patients in a hospital in Guiyang from December 2022 to February 2023 were investigated.The study included 209 middle-aged and elderly patients with a confirmed COVID-19 diagnosis.They were divided into a death group(n=26)and a survival group(n=183).Data were obtained retrospectively to analyse the relationship between test indicators to PLR and patient death.Results A total of 209 patients,114(60.6%)of whom were male.The total mean age of the patients was 78.02±12.59 years.Multifactorial linear regression results showed that PLR was independently associated with in-hospital death after adjusting for potential confounders[HR=1.18(1.02,1.38),P=0.0303],consistent with the results for PLR(quartiles)as a categorical variable(P=0.038).Curve fit analysis showed a non-linear relationship between PLR and in-hospital mortality with an inflection point value of 540.The HR and confidence intervals to the left and right of the inflection point were 2.0(1.4,2.9)and 0.9(0.7,1.1)respectively.Conclusion Elevated platelet/lymphocyte(PLR)was non-linearly related to mortality and positively associated with in-hospital mortality when PLR<540.
