摘要
目的调查软式内镜清洗消毒质量的影响因素,并制定针对性干预对策。方法采用现场调研和抽检方式对九江市第一人民医院2023年1月-2023年12月清洗消毒的软式内镜共120镜次进行调查,采用内镜清洗消毒现状调研表以及三磷酸腺苷(ATP)生物硬荧光法对软式内镜清洗消毒质量实施检测,然后构建Logistic模型,分析软式内镜清洗消毒质量的影响因素。结果120镜次软式内镜抽检结果显示,清洗不合格8镜次,占比6.67%,消毒不合格4镜次,占比3.33%,清洗消毒合格108镜次,占比90.00%;不合格组临时聘用专职清洗人员、手工清洗软式内镜、无清洗消毒质量监测以及消毒人员防护未达标占比高于合格组,差异有统计学意义(P<0.05);构建Logistic多元回归模型,结果显示,临时聘用专职清洗人员、手工清洗软式内镜、无清洗消毒质量监测以及消毒人员防护情况未达标是软式内镜清洗消毒质量的影响因素(0R>1,P<0.05)。结论临时聘用专职清洗人员、手工清洗软式内镜、无清洗消毒质量监测以及消毒人员防护情况未达标是软式内镜清洗消毒质量的影响因素。
Objective To investigate the influencing factors on the quality of cleaning and disinfection of flexible endoscopes,and to develop targeted intervention strategies.Method A total of 120 soft endoscopes that were cleaned and disinfected in our hospital from January 2023 to December 2023 were investigated using on-site investigation and sampling methods.The current status of endoscope cleaning and disinfection was investigated using a survey table and adenosine triphosphate(ATP)biological hard fluorescence method to measure the quality of soft endoscope cleaning and disinfection.Then,a Logistic model was constructed to analyze the factors affecting the quality of soft endoscope cleaning and disinfection.Result The results of 120 rounds of soft endoscope sampling showed that 8 rounds of cleaning failed,accounting for 6.67%,4 rounds of disinfection failed,accounting for 3.33%,and 108 rounds of cleaning and disinfection failed,accounting for 90.00%;the proportion of temporary hiring of full-time cleaning personnel,manual cleaning of soft endoscopes,non cleaning and disinfection quality monitoring,and inadequate protection of disinfection personnel in the unqualified group was higher than that in the qualified group,and the difference was statistically significant(P<0.05);constructing a Logistic multiple regression model,the results showed that the influencing factors of temporary hiring of dedicated cleaning personnel,manual cleaning of soft endoscopes,not monitoring of cleaning and disinfection quality,and defective protection of disinfection personnel on the quality of soft endoscope cleaning and disinfection(OR>1,P<0.05).Conclusion Factors affecting the quality of cleaning and disinfection of soft endoscopes include temporary hiring of dedicated cleaning personnel,manual cleaning of soft endoscopes,not monitoring of cleaning and disinfection quality,and defective protection of disinfection personnel.
