脑-硬脑膜-颞浅动脉血管融通术治疗烟雾病和颅内动脉粥样硬化性疾病的研究进展

摘要 脑-硬脑膜-颞浅动脉血管融通术(encephaloduroarteriosynangiosis,EDAS)是一种间接血运重建技术,1979年由日本学者松岛善治先生发明[1],目前已广泛应用于烟雾病(moyamoya disease,MMD)治疗,尤其是儿童MMD。EDAS是将颞浅动脉与其周围的筋膜分离出来后,将其缝合在硬脑膜与脑表面贴敷,最终实现颈外-颈内动脉血运重建。与直接血管重建术相比,EDAS具有多种优势:①手术难度较低;②术中无需暂时阻断颅内血管;③手术并发症少;④仅在大脑需要时才逐渐进行血运重建,从而降低过度灌注带来水肿和出血的风险[2,3]。本文对EDAS的治疗机制、现状及评价方法进行文献总结,旨在为临床提供参考。
