摘要
目的通过对地区食管癌的不同影像资料进行分析,论证氟代脱氧葡萄糖(^(18)F-Fludeoxyglucose,^(18)F-FDG)和正电子发射型断层扫描(Positron Emission Tomography/Computed Tomography,PET/CT)对该类疾病的诊断价值。方法回顾性分析2021年3月10日—2023年12月24日就诊于内蒙古自治区人民医院的35例临床初诊为食管癌的患者。对患者进行^(18)F-FDG PET/CT检查,所得结果与病理学结果相比较,探讨^(18)F-FDG和PET/CT对该类疾病的诊断价值。结果^(18)F-FDG PET/CT显像对食管癌原发灶诊断准确率为85.8%,灵敏度为97.7%,特异性为60.0%。CT对食管癌原发灶诊断准确率为80.0%,灵敏度为92.9%,特异性为42.9%。对^(18)F-FDG PET/CT诊断结果与病理结果进行Kappa检验,检验结果一致性良好。^(18)F-FDG PET/CT诊断鳞癌准确率为63.4%、灵敏度为73.7%、特异性为100%,^(18)F-FDG PET/CT诊断腺癌准确率为75%、灵敏度为66.7%、特异性为100%。^(18)F-FDG PET/CT诊断腺癌的准确率较鳞癌高。鳞癌患者局部复发病灶最大SUV值(SUV_(max)值)高于腺癌患者局部复发病灶,差异有统计学意义(P<0.05)。食管癌患者^(18)F-FDG PET/CT淋巴结转移灶与远处转移灶的SUV_(max)值比较差异无统计学意义(P>0.05)。结论^(18)F-FDG PET/CT对于食管癌的鉴别以及食管癌术后临床分期等方面能够提供可靠的半定量化诊断价值,而且SUV_(max)的表达对于食管癌术后患者的复发转移情况有着重要的参考价值,从而可以更好的指导临床治疗。
Objective By analyzing different imaging data of esophageal cancer in different regions,this study demonstrates the diagnostic value of ^(18)F Fluorodeoxyglucose(^(18)F FDG)and positron emission tomography(PET/CT)for this type of disease.Methods A retrospective analysis was performed on 35 patients with esophageal cancer who were admitted to the People's Hospital of Inner Mongolia Autonomous Region from March 10,2021 to December 24,2023.The patients were examined by ^(18)F-FDG PET/CT,and the results were compared with the pathological results to explore the diagnostic value of ^(18)F-FDG and PET/CT in this kind of diseases.Results The accuracy,sensitivity,and specificity of ^(18)F-FDG PET/CT imaging in the diagnosis of primary esophageal cancer were 85.8%,97.7%,and 60.0%,respectively.The diagnostic accuracy of computed tomography(CT)for primary esophageal cancer was 80.0%,with a sensitivity of 92.9%and a specificity of 42.9%.The Kappa test was performed on the diagnostic and pathological results of ^(18)F-FDG PET/CT,and the consistency of the test results was good.The accuracy,sensitivity,and specificity of ^(18)F-FDG PET/CT in diagnosing squamous cell carcinoma were 63.4%,73.7%,and 100%,respectively.The accuracy,sensitivity,and specificity of ^(18)F-FDG PET/CT in diagnosing adenocarcinoma were 75%,66.7%,and 100%,respectively.The accuracy of ^(18)F-FDG PET/CT in diagnosing adenocarcinoma was higher than that of squamous cell carcinoma.The maximum SUV value(SUV_(max)value)of locally recurrent lesions in squamous cell carcinoma patients was higher than that of locally recurrent lesions in adenocarcinoma patients,with a statistical difference(P<0.05).There was no statistically significant difference in SUV_(max)values between ^(18)F-FDG PET/CT lymph node metastases and distant metastases in esophageal cancer patients(P>0.05).Conclusion ^(18)F-FDG PET/CT can provide reliable semi quantitative diagnostic value for the differentiation of esophageal cancer and postoperative clinical staging of esophageal cancer.Moreover
