^(18)F-FDG PET/CT对肺腺癌实体/微乳头成分的预测价值

Predictive Value of ^(18)F-FDG PET/CT for Solid/Micro Papillary Component of Lung Adenocarcinoma

摘要 目的 探讨^(18)F-脱氧葡萄糖(FDG)正电子发射计算机断层扫描(PET/CT)最大标准化摄取值(SUV_(max))、肿瘤代谢体积(MTV)、病灶糖酵解总量(TLG)对肺腺癌实体/微乳头成分的预测价值。方法 回顾性搜集73例经病理证实为浸润性肺腺癌且病灶直径≤3 cm患者的临床病理资料及术前^(18)F-FDG PET/CT代谢参数,根据病理组织学亚型分为含实体/微乳头成分M/P+组与不含实体/微乳头成分M/P-组,单因素分析中各参数在M/P+组与M/P-组间的差异比较采用Mann-Whitney U检验或χ^(2)检验,采用多因素Logistic回归分析肺腺癌实体/微乳头成分存在的独立影响因素,采用受试者工作特征(ROC)曲线分析其预测效能。结果 单因素分析发现,SUV_(max)(P=0.002)、淋巴结转移(P=0.029)、肿瘤分期(P=0.011)与M/P+组相关,MTV(P=0.462)、TLG(P=0.188)、性别(P=0.239)、吸烟史(P=0.158)、年龄(P=0.193)、CEA水平(P=0.205)在两组间差异无统计学意义。多因素分析结果表明,SUV_(max)是肺腺癌实体/微乳头成分存在的独立影响因素(OR:1.209,95%CI:1.035~1.411;P=0.016)。ROC曲线分析显示,SUV_(max)预测肺腺癌病灶存在实体/微乳头成分的曲线下面积为0.710(95%CI:0.589~0.830;P=0.002),SUV_(max)预测的最佳截断值为8.10。结论 SUV_(max)对肺腺癌原发灶含有实体/微乳头成分具有较好的预测效能,并且SUV_(max)是预测肺腺癌实体/微乳头成分存在的独立影响因素。 Objective To investigate the predictive value of 18 F-deoxyglucose(FDG)PET/CT maximum standardized uptake value(SUV_(max)),tumor metabolic volume(MTV),and total focal glycolysis(TLG)on the solid/micro papillary component of lung adenocarcinoma.Methods The clinicopathological data and preoperative 18 F-FDG PET/CT metabolic parameters of 73 patients with pathologically confirmed invasive lung adenocarcinoma with lesions≤3 cm in diameter were retrospectively collected and divided into M/P+group with solid/micropapillary component and M/P-group without solid/micropapillary component according to pathological histological subtypes,and the differences of each parameter between M/P+and M/P-groups in univariate analysis were compared using Mann-Whitney U test orχ^(2) test,and independent in-fluences on the presence of solid/micropapillary component in lung adenocarcinoma were analyzed using multifactorial logistic regression,and their predictive efficacy was analyzed using subject operating characteristic(ROC)curves.Results Univariate analysis revealed that SUV_(max)(P=0.002),lymph node metastasis(P=0.029),and tumor stage(P=0.011)were associated with the M/P+group,and MTV(P=0.462),TLG(P=0.188),gender(P=0.239),smoking history(P=0.158),age(P=0.193),CEA levels(P=0.205)were not statistically different between the two groups.Multifactorial analysis showed that SUV_(max) was an independent influence on the presence of solid/micropapillary component in lung adenocarcinoma(OR:1.209,95%CI:1.035-1.411;P=0.016).ROC curve analysis showed that the area under the curve(AUC)of SUV_(max) predicting the presence of solid/micropapillary component in lung adenocarcinoma lesions was,and the best cut-off value for SUV_(max) prediction was 8.10.Conclusion SUV_(max) had better predictive efficacy for primary lung adenocarcinoma lesions containing solid/micropapillary components,and SUV_(max) was an independent influential factor in predicting the presence of solid/micropapillary components in lung adenocarcinoma.
