摘要
目的探讨快速康复外科(ERAS)理念下护理对腹腔镜胆囊切除术患者术后疼痛、应对方式的影响。方法选择2022年10月-2023年10月接收的110例腹腔镜胆囊切除术患者,按随机数字表法纳入对照组(n=55,辅以常规围术期护理)、观察组(n=55,辅以ERAS理念下护理)。比较两组术后并发症、术后恢复时间、术后视觉模拟量表(VAS)评分、简易应对方式问卷(SCSQ)评分、消化道相关疾病生命质量指数(GIQLI)评分。结果观察组术后伤口感染、皮下气肿、腹腔出血、恶心呕吐等并发症比对照组少,差异有统计学意义(P<0.05)。观察组术后肠鸣音恢复时间、初次摄食时间、初次下床时间、总住院时间比对照组短,差异有统计学意义(P<0.05)。观察组术后2h、12h、48h的VAS评分比对照组低,差异有统计学意义(P<0.05)。护理前比较,两组积极与消极应对评分、GIQLI总分差异无统计学意义(P>0.05);护理后比较,两组积极应对评分、GIQLI总分升高,消极应对评分降低,并且观察组积极应对评分、GIQLI总分比对照组高,消极应对评分比对照组低,差异有统计学意义(P<0.05)。结论腹腔镜胆囊切除术患者实施基于ERAS理念下护理能够减少术后并发症,减轻患者疼痛感受,加快术后恢复进程,纠正患者应对方式,提高生活质量。
Objective To observe the effects of nursing under the concept of rapid rehabilitation surgery(ERAS)on postoperative pain and coping styles in patients undergoing laparoscopic cholecystectomy.Methods From October 2022 to October 2023,110 patients with laparoscopic cholecystectomy were enrolled into the control group(n=55,supplemented with routine perioperative care)and observation group(n=55,supplemented with care under ERAS concept)according to random number table method.Postoperative complications,postoperative recovery time,postoperative visual analogue Scale(VAS)score,Simple Coping Style Questionnaire(SCSQ)score and digestive tract related disease Quality of Life index(GIQLI)score were compared.Results Postoperative complications such as wound infection,subcutaneous emphysema,abdominal hemorrhage,nausea and vomiting were less in the observation group than in the control group(P<0.05).The recovery time of bowel sound,the first feeding time,the first getting out of bed time and the total hospital stay in the observation group were shorter than those in the control group(P<0.05).The VAS scores of the observation group at 2h,12h and 48h after operation were lower than those of the control group(P<0.05).Before nursing,there was no significant difference in positive and negative coping scores and GIQLI scores between the two groups(P>0.05).After nursing comparison,the positive coping score and GIQLI total score of the two groups increased,while the negative coping score and GIQLI total score of the observation group were higher than those of the control group,and the negative coping score was lower,with statistical significance(P<0.05).Conclusion The implementation of ERAS based nursing in laparoscopic cholecystectomy patients can reduce postoperative complications,alleviate patients'pain perception,accelerate the postoperative recovery process,correct patients'coping style,and improve the quality of life.
