摘要
目的探究影响腹膜透析清除β2微球蛋白(β2-MG)的相关因素。方法回顾性选取2021年6月至2022年12月晋城市人民医院收治的112例行腹膜透析的患者为研究对象。收集患者基线资料(性别、年龄、透析龄、原发病)、腹膜转运功能、实验室指标[残余尿量、尿素氮清除率(Kt/V)、甘油三酯、总胆固醇、血磷、血钙、血尿酸、血肌酐、胱抑素C(CysC)、超敏C反应蛋白(hs-CRP)、血红蛋白、血清铁蛋白(SF)、白蛋白、甲状旁腺素(PTH)],并检查患者血清β2-MG水平。采用单因素分析及多元线性回归分析分析影响患者β2-MG清除的相关因素。结果单因素分析显示,不同性别、年龄、原发病、甘油三酯、总胆固醇、血钙、血尿酸、血红蛋白、SF、白蛋白、PTH的腹膜透析患者血清β2-MG水平比较,差异均无统计学意义(P>0.05);不同透析龄、腹膜转运功能、残余尿量、Kt/V、血磷、血肌酐、CysC、hs-CRP的腹膜透析患者血清β2-MG水平比较,差异均有统计学意义(P<0.05)。多元线性回归分析显示,透析龄、腹膜转运功能、残余尿量、血肌酐、CysC、hs-CRP为影响腹膜透析清除β2-MG的相关因素(OR=2.467,95%CI:1.325~4.592;OR=3.861,95%CI:11.679~8.882;OR=0.156,95%CI:0.040~0.612;OR=10.837,95%CI:2.967~39.589;OR=1.062,95%CI:1.019~1.106;OR=5.463,95%CI:1.649~18.094;P<0.05)。结论透析龄、腹膜转运功能、残余尿量、血肌酐、CysC、hs-CRP均为影响腹膜透析清除β2-MG的相关因素。
Objective To explore the related factors affecting peritoneal dialysis clearance ofβ2-microglobulin(β2-MG).Methods A total of 112 peritoneal dialysis patients admitted to Jincheng People's Hospital from June 2021 to December 2022 were retrospectively selected as the study objects.The baseline data(gender,age,dialysis age,primary disease),peritoneal transport function,laboratory indicators[residual urine volume,urea nitrogen clearance rate(Kt/V),triglycerides,total cholesterol,blood phosphorus,blood calcium,blood uric acid,blood creatinine,cystatin C(CysC),hypersensitive C-reactive protein(hs-CRP),hemoglobin,serum ferritin(SF),albumin,parathyroid hormone(PTH)]were collected,and the level of serumβ2-MG was detected.And the related factors affecting the clearance ofβ2-MG were analyzed by univariate analysis and multivariate linear regression analysis.Results Univariate analysis showed that there were no statistically significant difference in serumβ2-MG levels among peritoneal dialysis patients of different genders,ages,primary diseases,triglycerides,total cholesterol,blood calcium,blood uric acid,hemoglobin,SF,albumin,and PTH(P>0.05);there were statistically significant difference in serumβ2-MG levels among peritoneal dialysis patients with different dialysis ages,peritoneal transport function,residual urine volume,Kt/V,blood phosphorus,blood creatinine,CysC,hs-CRP(P<0.05).Multivariate linear regression analysis showed that dialysis age,peritoneal transport function,residual urine volume,blood creatinine,CysC,hs-CRP were related factors affecting the clearance ofβ2-MG in peritoneal dialysis(OR=2.467,95%CI:1.325-4.592;OR=3.861,95%CI:11.679-8.882;OR=0.156,95%CI:0.040-0.612;OR=10.837,95%CI:2.967-39.589;OR=1.062,95%CI:1.019-1.106;OR=5.463,95%CI:1.649-18.094;P<0.05).Conclusion Dialysis age,peritoneal transport function,residual urine volume,blood creatinine,CysC,hs-CRP are all related factors affecting the clearance ofβ2-MG in peritoneal dialysis.
