摘要
目的分析唑来膦酸结合石膏固定治疗老年桡骨远端骨折合并骨质疏松症(OP)患者骨折端骨折线X线评分(RUSS)、双能X线吸收仪(DXA)评分及临床效果。方法选取西安市第八医院2019年4月至2022年5月收治的98例老年骨质疏松性桡骨远端骨折患者,按治疗方法将其分为对照组49例、观察组49例。所有患者均给予石膏固定、骨质疏松干预及手法复位等治疗,观察组在此基础上联合给予唑来膦酸注射治疗,对比2组患者RUSS、DXA动态评分及骨折愈合时间,治疗60 d后进行复诊,采用Gart-land-Werley腕关节功能评分评价,治疗90 d后对患者进行随访,采用腕关节患者自行评估量表(PRWE),对比2组患者预后情况。结果对照组患者骨折愈合时间为(5.9±1.2)周,观察组患者愈合时间为(5.8±1.1)周(P>0.05);观察组患者第2、4、6周RUSS评分高于对照组(P<0.05);对照组Gart-land-Werley得分(8.3±3.0)分,观察组得分(6.4±2.6)分,对照组PRWE得分(19.6±6.0)分,观察组得分(14.3±4.6)分,2组比较差异有统计学意义(P<0.05)。治疗90 d后,观察组患者的骨密度较对照组提高(P<0.05)。结论唑来膦酸联合手法整复石膏固定可提高老年桡骨远端骨折患者骨密度,改善骨质疏松情况,提升临床疗效和患者预后。
Objective To explore the RUSS and DXA scores and clinical effects of zoledronic acid combined with plaster fixation in the treatment of elderly patients with distal radius fractures and OP.Methods A total of 98 elderly patients with osteoporotic distal radius fractures admitted to Xi′an Eighth Hospital from April 2019 to May 2022 were selected and divided into a control group(49 cases)and an observation group(49 cases)by treatment methods.All patients were treated with plaster fixation,osteoporosis intervention,and manual reduction.On this basis,the observation group was treated with zoledronic acid injection in combination,and the RUSS,DXA dynamic evaluation scores,and fracture healing time of the two groups of patients were compared.After 60 days of treatment,follow-up was conducted using the Gart land Werley wrist joint function score.After 90 days of treatment,patients were followed up and the prognosis of the two groups of patients was compared using the PRWE patient self-evaluation scale.Results The fracture healing time in the control group was(5.9±1.2)weeks,while in the observation group it was(5.8±1.1)weeks(P>0.05).The RUSS score of the observation group was significantly higher than that of the control group at weeks 2,4,and 6(P<0.05).The control group had a Gart-land-Werley score of(8.3±3.0),while the observation group had a score of(6.4±2.6).The PRWE score in the control group was(19.6±6.0),while the score in the observation group was(14.3±4.6)(P<0.05).After 90 days of treatment,the bone density of the observation group was significantly increased compared to the control group(P<0.05).Conclusion Zoledronic acid combined with manual reduction and plaster fixation can increase bone density,improve osteoporosis,enhance clinical efficacy,regulate bone score,have high safety,and have a good prognosis in elderly patients with distal radius fractures.
