摘要
目的 分析探究三维能量多普勒血管化参数在胎儿生长受限(FGR)中的预测效能。方法选取2021年12月至2022年12月期间在我院进行孕检及分娩的单胎妊娠孕妇162例作为研究对象,所有孕妇均在产检时接受三维能量多普勒超声检测。根据分娩后胎儿诊断结果,将有生长受限胎儿设为FGR组(110例),将生长未受限胎儿设为对照组(52例)。对比2组胎儿临床生长指标(头围、胎盘商、胎盘体积、股骨长度、腹围、头顶径)、血管化参数[血流指数(FI)、血管化指数(VI)、血管化-血流指数(VFI)]、分析胎儿临床生长指标与血管化参数相关性,通过受试者工作特征(ROC)曲线评估血管化参数对FGR的预测效能。结果FGR组头围、胎盘商、胎盘体积、股骨长度、腹围、头顶径明显低于对照组,差异均有统计学意义(P<0.05);对照组FI、VI、VFI均显著高于FGR组,差异均有统计学意义(P<0.05);Pearson相关分析显示,FI、VI、VFI与头围、胎盘商、胎盘体积、股骨长度、腹围、头顶径均呈正相关(P均<0.05);ROC曲线分析显示,对于FGR的预测,FI、VI、VFI均有较高效能,且联合检测的ROC曲线下面积(AUC)以及灵敏度、特异度均高于各单项预测价值(P<0.05)。结论 三维能量多普勒超声检查显示血管化参数与FGR具有一定关系,血管化参数对临床预测FGR有较高的效能,有助于早期诊断并及时采取干预措施,值得临床推广应用。
Objective To investigate the predictive efficacy of three-dimensional energy Doppler vascularization parameters in fetal growth restriction(FGR).Methods A total of 162 pregnant women who underwent pregnancy examination and delivery in our hospital from December 2021 to December 2022 were selected as the research subjects.All pregnant women received three-dimensional energy Doppler ultrasound detection during delivery examination.According to the fetal diagnosis after delivery,the fetuses with growth restriction were set as FGR group(n=110),and the fetuses without growth restriction were set as control group(n=52).The clinical growth indicators(head circumference,placenta circumference,placenta volume,femur length,abdominal circumference,and head diameter)and vascularization parameters(FI,VI,VFI)of the two groups were compared,and the correlation between the clinical growth indicators and vascularization parameters of the fetus was analyzed.The predictive efficacy of vascularization parameters for FGR was evaluated by ROC curve.Results Head circumference,placenta volume,femur length,abdominal circumference and head diameter in FGR group were significantly/ower than those in control group,with statistical significance(P<0.05).FI,VI and VFI in control group were significantly higher than those in FGR group,with statistical significance(P<0.05).Pearson correlation analysis showed that FI,VI and VFI were positively correlated with head circumference,placental quotient,placental volume,femur length,abdominal circumference and head diameter(all P<0.05).ROC curve analysis showed that FI,VI and VFI had higher efficiency for FGR prediction,and the area under ROC curve(AUC),sensitivity and specificity of combined detection were higher than each single prediction value(P<0.05).Conclusion Three-dimensional energy Doppler ultrasound showed that vascularization parameters had a certain relationship with FGR,and vascularization parameters had high efficacy in clinical prediction of FGR,which was helpful for early diagnosis and t
