摘要
探讨系统免疫炎症指标(systemic immune-inflammation index,SII)与慢性鼻-鼻窦炎合并鼻息肉(chronic rhi-nosinusitis with nasal polyps,CRSwNP)患者行功能性鼻内镜手术(functional endoscopic sinus surgery,FESS)后复发的关系.选取2019年5月-2021年6月在平南县第二人民医院接受FESS治疗的120例CRSwNP患者为研究对象,其中失访14例,最终纳入106例进行数据分析.依据术后12个月内患者是否出现疾病复发,将患者分为未复发组(73例)和复发组(33例).比较两组患者一般资料,通过logistics回归评估CRSwNP术后复发的危险因素,采用受试者工作特征曲线(ROC)评估SII预测术后复发的价值.结果显示,复发组患者的外周血SII、EOS、鼻中隔偏曲占比明显升高(P<0.05).logistics回归分析提示外周血SII、EOS是术后复发的独立危险因素,SII预测CRSwNP术后复发的曲线下面积(AUC)为0.772,95%CI为0.678~0.866,敏感性为87.9%,特异性为53.4%.研究发现,CRSwNP患者外周血SII、EOS与FESS术后复发密切相关,对评估CRSwNP术后复发有一定的临床意义.
Exploring the relationship between systemic immune inflammation index(SII)and recurrence after functional endoscopic sinus surgery(FESS)in patients with chronic rhinosinusitis complicated with nasal polyps(CRSwNP).120 CRSwNP patients who received FESS treatment at the Second People’s Hospital of Pingnan from May 2019 to June 2021 were selected as the study subjects,with 14 cases lost to follow-up.Finally,106 cases were included for data analysis.According to whether the patient experienced disease recurrence within 12 months after surgery,the patients were divided into a non recurrence group(73 cases)and a recurrence group(33 cases).Compare the general information of two groups of patients,evaluate the risk factors for postoperative recurrence of CRSwNP through logistic regression,and evaluate the value of SII in predicting postoperative recurrence using receiver operating characteristic curve(ROC).The results showed that the proportion of peripheral blood SII,EOS,and nasal septum deviation in the recurrent group patients was significantly increased(P<0.05).Logistic regression analysis suggests that peripheral blood SII and EOS are independent risk factors for postoperative recurrence.The area under the curve(AUC)of SII predicting postoperative recurrence of CRSwNP is 0.772,with a 95%CI of 0.678~0.866,a sensitivity of 87.9%,and a specificity of 53.4%.Research has found that peripheral blood SII and EOS in CRSwNP patients are closely related to postoperative recurrence of FESS,and have certain clinical significance in evaluating postoperative recurrence of CRSwNP.
