摘要
目的 探讨甲状腺癌根治术患者于围术期接受手术室个性化舒适护理联合常规护理干预对生命体征、焦虑和抑郁情绪、疼痛与睡眠质量的影响。方法 方便选取2021年1月—2023年5月于南京中医药大学附属南京医院(南京市第二医院)就诊的149例甲状腺癌根治术患者作为研究对象,根据随机数表法分为两组,对照组(n=75)采用围术期常规护理干预,观察组(n=74)采用常规护理干预联合围术期手术室个性化舒适护理干预。比较两组患者生命体征(心率、收缩压)、不良情绪(焦虑自评量表评分、抑郁自评量表评分)、疼痛程度(视觉模拟评分法评分)、睡眠质量(匹兹堡睡眠质量评分)。结果 术前15 min,观察组心率(86.57±4.34)次/min、收缩压(129.51±6.52)mmHg低于对照组的(92.95±5.52)次/min、(134.23±5.36)mmHg,差异有统计学意义(t=7.848、4.830,P均<0.05)。护理后,两组焦虑、抑郁评分均明显降低,且观察组低于对照组,差异有统计学意义(P均<0.05)。术后1、2、3 d,观察组疼痛评分均低于对照组,差异有统计学意义(P均<0.05)。术后1、2、3 d,观察组睡眠质量评分均低于对照组,差异有统计学意义(P均<0.05)。结论 甲状腺癌根治术患者于围术期接受手术室个性化舒适护理,可改善围术期生命体征,减轻焦虑、抑郁情绪以及术后疼痛感,提升睡眠质量。
Objective To investigate the effects of personalized comfort nursing combined with routine nursing inter⁃vention on vital signs,anxiety and depression,pain and sleep quality in patients undergoing radical thyroidectomy.Methods A total of 149 patients treated in Nanjing Hospital Affiliated to Nanjing University of Traditional Chinese Medicine(Nanjing Second Hospital)from January 2021 to May 2023 were conveniently selected as the study objects.They were divided into two groups according to random number table method.The control group(n=75)was treated with perioperative routine nursing intervention,and the observation group(n=74)was treated with routine nursing in⁃tervention combined with perioperative personalized comfort nursing intervention in operating room.The vital signs(heart rate,systolic blood pressure),adverse mood(scores of Self-rating Anxiety Scale and Self-rating Depression Scale),pain degree(scores of Visual Analog Scale)and sleep quality(scores of Pittsburgh Sleep Quality Index)of the two groups were compared.Results 15 min before surgery, the heart rate (86.57±4.34) times/min and systolic blood pressure (129.51±6.52) mmHg in the observation group were lower than those in the control group [(92.95±5.52) times/ min and (134.23±5.36) mmHg], and the differences were statistically significant (t=7.848, 4.830, both P<0.05). After nursing, anxiety and depression scores of both groups were significantly decreased, and the observation group was lower than the control group, the differences were statistically significant (all P<0.05). On the 1st, 2nd and 3rd day after operation, the pain scores of the observation group were lower than those of the control group, and the differences were statistically significant (all P<0.05). On the 1st, 2nd and 3rd day after operation, the sleep quality score of the observation group were lower than that of the control group, and the differences were statistically significant (all P<0.05).
