摘要
目的分析肝硬化失代偿期患者外周血白细胞衍生性指标变化与免疫功能的相关性。方法选取2020年3月至2022年5月鹰潭市人民医院收治的80例肝硬化患者作为研究对象,其中代偿期患者45例(代偿组),失代偿期患者35例(失代偿组),另选取同期健康体检者30名作为健康组。比较3组肝功能指标、外周血白细胞衍生性指标及免疫功能,并采用Person分析外周血白细胞衍生性指标与肝功能、免疫功能的相关性。结果谷丙转氨酶(ALT)、谷草转氨酶(AST)、血清总胆红素(TBIL)水平失代偿组>代偿组>健康组,血清白蛋白(ALB)水平失代偿组<代偿组<健康组,3组及组内两两比较差异有统计学意义(P<0.05);中性粒细胞、淋巴细胞比值(NLR),衍生的中性粒细胞、淋巴细胞比值(dNLR),单核细胞、淋巴细胞比值(MLR),系统免疫炎症指数(SII)失代偿组>代偿组>健康组,血小板、淋巴细胞比值(PLR)失代偿组<代偿组<健康组,3组及组内两两比较差异有统计学意义(P<0.05);CD3^(+)、CD4^(+)含量失代偿组<代偿组<健康组,CD8^(+)、IgA、IgG、IgM失代偿组>代偿组>健康组,3组及组内两两比较差异有统计学意义(P<0.05);Person相关性分析结果显示,NLR、dNLR、MLR、SII与ALT、AST、TBIL、CD3^(+)、CD4^(+)呈正相关(r>0,P<0.05),与ALB、CD8^(+)、IgA、IgG、IgM呈负相关(r<0,P<0.05),PLR与ALT、AST、TBIL、CD3^(+)、CD4^(+)呈负相关(r<0,P<0.05),与ALB、CD8^(+)、IgA、IgG、IgM呈正相关(r>0,P<0.05)。结论肝硬化失代偿期患者外周血白细胞衍生性指标明显异常,免疫功能明显下降,且外周血白细胞衍生性指标与患者肝硬化肝功能、免疫功能具有相关性,建议临床密切监测。
Objective To analyze the correlation between changes in peripheral blood leukocyte-derived markers and immune function in patients with decompensated cirrhosis.Methods A total of 80 patients with liver cirrhosis who were admitted to Yingtan City People's Hospital from March 2020 to May 2022 were selected as the study subjects,including 45 patients in compensated stage(compensated group)and 35 patients in decompensated stage(decompensated group),30 healthy individuals were selected as the healthy group.The liver function indexes,peripheral blood leukocyte-derived indexes and immune function were compared among the three groups,Person correlation analysis was used to analyze the correlation between peripheral blood leukocyte-derived indexes and liver function and immune function.Results The levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and serum total bilirubin(TBIL)in the decompensated group>the compensated group>the healthy group,and the serum albumin(ALB)in the decompensated group<the compensated group<the healthy group;there were significant differences between the three groups and between the two groups(P<0.05).Neutrophil to lymphocyte ratio(NLR),derived neutrophil to lymphocyte ratio(dNLR),monocyte to lymphocyte ratio(MLR),systemic immune-inflammation index(SII)the decompensation group>the compensation group>the healthy group,platelet to lymphocyte ratio(PLR)the decompensation group<the compensation group<the healthy group,there were significant differences among the three groups and between the two group(P<0.05).The contents of CD3^(+)and CD4^(+)in the decompensation group<the compensation group<the healthy group,and the contents of CD8^(+),IgA,IgG and IgM in the decompensation group>the compensation group>the healthy group;there was significant difference among the three groups and between the two group(P<0.05);Person correlation analysis showed that NLR,dNLR,MLR and SII were positively correlated with ALT,AST,TBIL,CD3^(+)and CD4^(+)(r>0,P<0.05),and negatively correlated with ALB,CD
