摘要
目的 研究骨科关节机器人辅助下直接前侧入路(DAA)-全髋关节置换术(THA)与传统后外侧入路(PLA)-THA近期疗效。方法 选取我院2023年1月至2023年8月84例股骨颈骨折患者,按随机数字表法分成观察组42例,对照组42例。两组均采THA治疗,观察组采用骨科机器人辅助下DAA,对照组采用传统手工PLA。观察患者手术时间、下地活动时间、住院时间、下肢等长度、切口长度、术中出血量、疼痛视觉模拟量表(VAS)、Harris评分及术后影像检查测量假体髋臼角度。结果 84例均获得随访,随访时间为3~6个月。观察组下地活动时间早于对照组(P <0.05),髋臼假体前倾角及外展角精准于对照组(P <0.05);观察组双下肢等长度优于对照组(P <0.05);观察组术后Harris及VAS评分优于对照组(P <0.05)。结论 关节机器人辅助下平卧位DAA-THA具有手术微创损伤小、出血量少,假体安装精准、牢靠、下地活动时间早特点。
Objective To study the short-term clinical efficacy of orthopedic joint robot assisted supine position direct anterior approach(DAA)-total hip arthroplasty(THA) and traditional posterolateral approach(PLA)-THA.Methods Atotal of 84 patients with femoral neck fractures in our hospital from January 2023 to August 2023 were selected and randomly divided into an observation group of 42 cases and a control group of 42cases using a random number table method.Both groups underwent THA,while the observation group received DAA assisted by orthopedic robots,while the control group received traditional manual PLA.Observe the patient's surgical time,time spent on ground activities,length of hospital stay,length of lower limb,incision length,intraoperative bleeding,pain visual analogue scale(VAS),Harris scale,and postoperative imaging to measure the hip angle of the prosthesis.Perform statistical analysis and evaluation of the data.Results All 84 cases were followed up for 3-6 months.The observation group had significantly earlier lower ground movement time(P<0.05),and the anterior inclination angle and abduction angle of the acetabular prosthesis were significantly more accurate(P<0.05);The observation group with equal length of both lower limbs was significantly better than the control group(P<0.05);The Harris and WAS scores before and after surgery were significantly better in the observation group than in the control group(P<0.05).Conclusions The joint robot assisted supine position DAA-THA has the characteristics of minimal surgical damage,minimal bleeding,precise and reliable prosthesis installation,and early mobilization time.It is a new surgical method that is worth promoting.
