摘要
目的:评估多层螺旋CT冠状面重建图像在尘肺诊断中的应用价值。方法:选取2022年7月—2023年6月青岛市即墨区人民医院收治的疑似尘肺病患者76例作为研究对象,所有患者接受高千伏胸部X线及多层螺旋CT冠状面重建图像检查。以病理检查结果为“金标准”,比较两种检查方法尘肺征象及并发症检出率,以及对尘肺诊断的灵敏度、特异度、准确率。结果:多层螺旋CT小阴影显示、小阴影聚集、大阴影内部结构、小叶间隔增厚、胸膜下透光带、肺门及纵隔淋巴结钙化检出率高于高千伏胸部X线,差异有统计学意义(P<0.05);多层螺旋CT灵敏度、准确率高于高千伏胸部X线,差异有统计学意义(P<0.05);两种检查方法特异度比较,差异无统计学意义(P>0.05);高千伏胸部X线对尘肺病患者肺气泡、支气管扩张、肺部感染、结核并发症的检出率均低于多层螺旋CT,差异有统计学意义(P<0.05)。结论:多层螺旋CT在尘肺病的诊断中灵敏度和准确率较高,并且在尘肺征象及其并发症检测中具有显著优势。
Objective:To evaluate the application value of multi-slice spiral CT coronal reconstruction images in the diagnosis of pneumoconiosis.Methods:A total of seventy-six patients with suspected pneumoconiosis treated in Qingdao Jimo District People's Hospital from July 2022 to June 2023 were selected as the study objects.All patients received high kV chest X-ray and multi-slice spiral CT coronal reconstruction image examination.With the pathological examination results as the"gold standard",the detection rate of pneumoconiosis signs and complications,and the sensitivity,specificity and accuracy of the diagnosis of pneumoconiosis were compared between the two methods.Results:The detection rates of small shadow display,small shadow aggregation,large shadow internal structure,lobular septal thickening,subpleural light transmittance band,hilum and mediastinal lymph node calcification on multi-slice spiral CT were higher than those on high kV chest X-ray,the difference was statistically significant(P<0.05);the sensitivity and accuracy of multi-slice spiral CT were higher than that of high kV chest X-ray,and the difference was statistically significant(P<0.05);there was no significant difference in the specificity between the two methods(P>0.05);the detection rates of pulmonary bubbles,bronchiectasis,pulmonary infection and tuberculosis in patients with pneumoconiosis with high kV chest X-ray were lower than those of multi-slice spiral CT,and the difference was statistically significant(P<0.05).Conclusion:Multi-slice spiral CT has high sensitivity and accuracy in the diagnosis of pneumoconiosis,and has significant advantages in the detection of pneumoconiosis signs and complications.
