摘要
目的 探究与分析老年人体质量指数(BMI)对早期血压、心脏结构功能改变的影响。方法 回顾性分析2020年5月—2022年9月在山东第一医科大学附属省立医院实施健康体检的108例老年人的临床资料,老年人均接受了身高、体质量的测量,同时对其血压以及心脏超声进行了监测,按照不同BMI分为了消瘦组(33例)、正常组(31例)、超重组(25例)以及肥胖组(19例),不同BMI患者的血压水平以及心脏功能结构指标比较进行方差分析;采用多元线性回归分析探讨老年人BMI与早期血压、心脏结构功能指标的相关性。结果 消瘦组、正常组、超重组以及肥胖组的收缩压、舒张压、左室质量指数(LVMI)、左心室舒张末壁厚度(LVPWTd)、左心室舒张壁厚度(LVPWs)、左心室收缩末期内径(LVESd)、左心室舒张末期内径(LVEDd)、室间隔厚度(IVSs)、左心室射血分数(LVEF)比较,差异均有统计学意义(P<0.05)。多元线性回归分析显示:BMI与收缩压、舒张压,LVMI、LVPWTd、LVPWs、LVESd、LVEDd、IVSs呈正相关(P<0.05),与LVEF呈负相关(P<0.05)。结论 不同BMI的老年人血压及心脏结构功能指标呈现出了明显的差异性;BMI水平对早期血压和心脏结构功能指标有显著影响,随着BMI的增加,老年人的血压及心脏结构功能会发生相应的改变。
Objective To investigate and analyze the effect of body mass index(BMI)on early changes of blood pressure and cardiac structure and function in the elderly.Methods The clinical data of 108 elderly patients who underwent health examination in the Provincial Hospital Affiliated to Shandong First Medical University from May 2020 to September 2022 were retrospectively analyzed.All patients received height and body mass measurement,and their blood pressure and cardiac ultrasound were monitored.They were divided into emaciation group(33 cases),normal group(31 cases),overweight group(25 cases)and obese group(19 cases)according to different BMI.The blood pressure levels and cardiac function structural indicators of patients with different BMI were compared for analysis of variance;multiple linear regression analysis was used to investigate the correlation between BMI and early blood pressure and cardiac structure and function indicators in the elderly.Results There were significant differences in systolic blood pressure,diastolic blood pressure,left ventricular mass index(LVMI),left ventricular posterior wall thickness of end-diastolic(LVPWTd),left ventricular posterior wall thickness at end-systole(LVPWs),left ventricular end-systolic diameter(LVESd),left ventricular end-diastolic diameter(LVEDd),interventricular septum thickness(IVSs),and left ventricular ejection fraction(LVEF)among the lean,normal,overweight,and obese groups(P<0.05).Multiple linear regression analysis showed that BMI was positively correlated with systolic and diastolic blood pressure,and LVMI,LVPWTd,LVESd,LVPWs,LVEDd,IVSs,and it was negatively correlated with LVEF(P<0.05).Conclusion The blood pressure and cardiac structure and function of the elderly with different BMI showed significant differences;BMI level had a significant effect on early blood pressure and cardiac structure and function indexes,and with the increase of BMI,the blood pressure and cardiac structure and function of the elderly would change correspondingly.
