摘要
目的编制肾移植术后患者自我管理调查问卷,并检测调查问卷效度。方法选取2018年1月—2020年1月于上海交通大学医学院附属瑞金医院普外科肝肾移植病区行肾移植手术的患者34例,调查问卷通过文献回顾形成初稿,邀请来自上海交通大学医学院附属瑞金医院移植中心临床及护理、药剂科、营养科、康复科8名专家,采用德尔菲法对该调查问卷进行2轮会议讨论。采用随机抽样法抽取上海交通大学医学院附属瑞金医院肝肾移植病区34例肾移植受者进行预试验调查,检验问卷的信效度,并形成最终版调查问卷。结果该调查问卷共42个条目,包括知识维度、态度维度和实践维度3个维度。共收集问卷34份,有效回收率为100%。问卷总体Cronbach'sα系数为0.923,知识维度的Cronbach'sα系数为0.931,态度维度的Cronbach'sα系数为0.706,实践维度(术后随访复查的依从性)的Cronbach'sα系数为0.898,Morisky用药依从性问卷的Cronbach'sα系数为-0.084,调查结果显示该问卷具有较好的内部一致性,知识维度、态度维度及实践维度信度均表现良好。结论肾移植受者术后自我管理相关的水平、态度及实践调查问卷具有良好的信效度,可用于肾移植术后受者自我管理水平现状的调查及评估,提供借鉴内容。
Objective To develop a questionnaire for assessing post-transplant self-management in kidney transplant recipients and to evaluate its validity.Methods A total of 34 patients who underwent kidney transplantation in liver and kidney transplantation center,department of general surgery,Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2018 to January 2020 were collected.The preliminary draft of the survey questionnaire was formed through literature review.8 experts from the clinical and nursing,pharmacy,nutrition,and rehabilitation departments of transplantation center,Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were invited to conduct two rounds of meeting discussions using the Delphi method.Using random sampling method,34 kidney transplant recipients from liver and kidney transplantation center,Ruijin Hospital Affiliated to Shanghai Jiao Tong University School of Medicine were selected for a preliminary trial survey to test the reliability and validity of the questionnaire,and the final version of the survey questionnaire was formed.Results The questionnaire comprised 42 items,encompassing three dimensions:knowledge,attitudes,and practices.A total of 34 valid responses were collected,achieving a 100%response rate.The overall Cronbach′sαcoefficient for the questionnaire was 0.923,withαvalues of 0.931 for knowledge,0.706 for attitudes,0.898 for practices(compliance with postoperative follow-up examinations),and-0.084 for the morisky medication adherence questionnaire.The survey results indicated good internal consistency,with strong reliability observed in the knowledge,attitudes,and practices dimensions.Conclusion The questionnaire on post-transplant self-management levels,attitudes,and practices for kidney transplant recipients demonstrates good reliability and validity.It can be utilized to investigate and assess the current status of self-management among post-kidney transplant recipients.
