摘要
回收式自体输血(IOCS)是临床中一种重要的血液保存方式,由于围手术期出血是影响患者手术进程、康复、甚至危及患者生命的重要因素,在血液需求量极大的脊柱外科中得到广泛的应用。本文作者对IOCS在脊柱外科的适应证和禁忌证、应用现状、血液质量、经济效益以及存在的问题(脊柱肿瘤手术中的应用、白细胞污染)研究现状及进展进行了综述。目前大量研究表明IOCS可以降低脊柱患者异体输血的需求,降低相关风险和成本,如果使用得当,与异体血液相比,自体抢救血液可能具有更高的质量,可降低术后并发症的概率并加速术后康复,符合加速康复理念,但在实际应用中尚存在一些问题,例如白细胞过滤、细菌污染或脊柱肿瘤手术中的应用,值得进行深度的研究和探索。
Intraoperative cell salvage(IOCS)is an important way of blood preservation in clinic.Because perioperative bleeding is an important factor affecting the process of operation,recovery,and even endangering the life of patients,it has been widely used in spinal surgery with great blood demand.The authoRthis article reviews the indications and contraindications,application status,blood quality,economic benefits and existing problems(application of spinal tumoRsurgery,leukocyte contamination)of IOCS in spinal surgery.At present,a large numbeRof studies have shown that IOCS can reduce the demand foRallogeneic blood transfusion in patients with spine,reduce the related risks and costs.If used properly,autologous rescue blood may have higheRquality than allogeneic blood,reduce the probability of postoperative complications and accelerate postoperative rehabilitation,which is in line with the concept of accelerated rehabilitation.However,there are still some problems in practical application,such as leukocyte filtration,bacterial contamination oRspinal tumoRsurgery,which are worthy of in-depth research and exploration.
