摘要
目的探讨振幅整合脑电图(a EEG)检查对低出生体重早产儿发生脑损伤的预测价值。方法选取2020年1月至2023年1月于临沂市妇幼保健院产科娩出的114例低出生体重早产儿作为研究对象,按照头颅核磁共振成像结果有无异常分为脑损伤组(n=29)和非脑损伤组(n=85)。比较两组a EEG结果,并进行多因素logistic回归分析,采用受试者工作特征曲线评价a EEG检查对低出生体重早产儿发生脑损伤的预测价值。结果脑损伤组不成熟的睡眠-觉醒周期(SWC)占比及a EEG异常率均高于非脑损伤组(P<0.05)。多因素logistic回归分析显示,不成熟的SWC、a EEG异常是低出生体重儿脑损伤的危险因素(P<0.05)。不成熟的SWC、a EEG异常联合检测的曲线下面积大于单一检测,灵敏度和特异度均高于单一检测(P<0.05)。结论a EEG检查对低出生体重早产儿发生脑损伤的预测价值较高,不成熟的SWC、a EEG异常是低出生体重儿脑损伤的重要预测指标。
Objective To investigate the predictive value of amplitude-integrated electroencephalogram(aEEG)for brain injury in premature infants with low birth weight.Methods A total of 114 premature infants with low birth weight who were delivered in the Department of Obstetrics of Linyi Maternal and Child Health Hospital from January 2020 to January 2023 were selected as the study objects.According to the results of head magnetic resonance imaging,they were divided into brain injury group(n=29)and non-brain injury group(n=85).The aEEG results of the two groups were compared,and multivariate logistic regression analysis was performed.The predictive value of aEEG for brain injury in premature infants with low birth weight was evaluated by receiver operating characteristic curve.Results The proportion of immature sleep-wake cycling(SWC)and the abnormal rate of aEEG in brain injury group were higher than those in non-brain injury group(P<0.05).Multivariate logistic regression analysis showed that immature SWC and aEEG abnormality were risk factors for brain injury in premature infants with low birth weight(P<0.05).The area under the curve of immature SWC and aEEG abnormality combined detection was larger than that of single detection,and the sensitivity and specificity were higher than those of single detection(P<0.05).Conclusion The examination of aEEG has high predictive value for brain injury in premature infants with low birth weight,and immature SWC and aEEG abnormality are important predictors of brain injury in premature infants with low birth weight.
