摘要
目的探究冲击量糖皮质激素在初诊特发性血小板减少性紫癜患者中的临床治疗效果。方法选取2020年1月—2023年6月江苏省建湖县人民医院收治的初诊特发性血小板减少性紫癜患者80例,信封法随机分成两组,每组40例。对照组常规治疗,观察组冲击量糖皮质激素治疗。对比两组治疗总有效率、免疫功能指标、相关临床指标、炎性因子指标、不良反应。结果观察组治疗总有效率为95.00%,相比对照组的77.50%显著更高,差异有统计学意义(χ^(2)=5.165,P<0.05)。观察组各项免疫功能指标水平均高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组肿瘤坏死因子-α、白细胞介素-6炎性指标水平及血小板计数、相容性人类白细胞抗原G指标水平均优于对照组,差异有统计学意义(P均<0.05)。观察组患者不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论冲击量糖皮质激素用于初诊特发性血小板减少性紫癜患者治疗中的效果相对理想,安全系数较高。
Objective To explore the clinical effect of impinged glucocorticoid in patients with idiopathic thrombocytopenic purpura.Methods A total of 80 patients with idiopathic thrombocytopenic purpura newly diagnosed admitted in Jianhu County People's Hospital of Jiangsu Province from January 2020 to June 2023 were randomly divided into two groups by envelope method,with 40 cases in each group.The control group was treated with routine treatment,and the observation group was treated with glucocorticoid.The total effective rate,immune function index,related clinical index,inflammatory factor index and adverse reaction were compared between the two groups.Results The total effective rate of the observation group was 95.00%,which was significantly higher than that of the control group(77.50%),and the differences were statistically significant(χ^(2)=5.165,P<0.05).The levels of immune function indexes in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of tumor necrosis factor-αand interleukin-6 inflammatory indexes,platelet count and compatible human leukocyte antigen G indexes in observation group were better than those in control group,and the differences were statistically significant(all P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,the difference was statistically significant(P<0.05).Conclusion The effect of impinged glucocorticoids in the treatment of newly diagnosed patients with idiopathic thrombocytopenic purpura is relatively ideal,and the safety factor is high.
