摘要
目的探究血必净注射液对行连续性肾脏替代疗法的多器官功能障碍综合征(multiple organ dysfunction syndrome,MODS)患者凝血功能的影响。方法将2018年3月~2023年3月我院收治的82例MODS患者采用随机数字表法分为研究组和对照组,各41例,对照组予以连续性肾脏替代疗法治疗,研究组在对照组治疗基础上联合血必净注射液治疗。治疗3 d后,比较两组患者的凝血酶时间(thrombin time,TT)、凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、白细胞介素-1β(interleukin-1,IL-1β)、肿瘤坏死因子α(tumor necrosis factor-alpha,TNF-α)、高迁移率族蛋白B1(high mobility group protein B1,HMGB-1)、肌酸激酶(creatine kinase,CK)、乳酸脱氢酶(lactate dehydrogenase,LDH)、肌酸激酶同工酶(creatine kinase isoenzyme,CK-MB)、总胆红素(total bilirubin,TBil)、天冬氨酸氨基转移酶(aspartate aminotransferase,AST)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)、血肌酐(serum creatinine,sCr)及尿素氮(urea nitrogen,BUN)水平,记录不良反应及MODS死亡率。结果治疗后,两组的PT、APTT均升高,研究组高于对照组(P<0.05);两组的IL-1β、TNF-α、HMGB-1、CK、LDH、CK-MB、TBil、AST、ALT均降低,研究组低于对照组(P<0.05)。两组的sCr、BUN水平比较,差异无统计学意义(P>0.05)。两组均未监测到严重不良反应。研究组MODS死亡率低于对照组(P<0.05)。结论血必净注射液联合连续性肾脏替代疗法治疗MODS,有利于减轻血液高凝状态,抑制炎症反应,改善患者的心、肝功能,且安全性良好。
Objectivee To explore the effect of Xuebijing injection on coagulation function in patients with multiple organ dysfunction syndrome(MODS)undergoing continuous renal replacement therapy.Methods From March 2018 to March 2023,82 patients with MODS admitted to our hospital were randomly divided into study group and control group,with 41 cases each.The control group was treated with continuous renal replacement therapy,and the study group was treated with Xuebijing injection on the basis of the treatment in the control group.After 3 days of treatment,the thrombin time(TT),prothrombin time(PT),activated partial thromboplastin time(APTT),interleukin-1β(IL-1β),tumor necrosis factor-alpha(TNF-α),high mobility group protein B1(HMGB-1),creatine kinase(CK),lactate dehydrogenase(LDH),creatine kinase isoenzyme(CK-MB),total bilirubin(TBil),aspartate aminotransferase(AST),alanine aminotransferase(ALT),serum creatinine(sCr)and urea nitrogen(BUN)levels were recorded,and adverse reactions and MODS mortality were recorded.Results After treatment,the PT and APTT in both groups increased,and the study group was higher than the control group(P<0.05).The levels of IL-1β,TNF-α,HMGB-1,CK,LDH,CK-MB,TBil,AST,and ALT in the study group were lower than those in the control group(P<0.05).There was no statistically significant difference in the levels of sCr and BUN between the two groups(P>0.05).No serious adverse reactions were detected in either group.The mortality rate of MODS in the study group was lower than that in the control group(P<O.05).Conclusion Xuebijing injection combined with continuous renal replacement therapy in the treatment of MODS is beneficial to reducing blood hypercoagulability,inhibiting inflammatory reactions,and improving patients'cardiac and liver functions,with good safety.
